About This Presentation

Description : Tumor Resection Techniques: Innovations and Practices in Surgical Oncology
Tumor resection is a cornerstone of surgical oncology, playing a pivotal role in the treatment of cancers. This presentation, “Tumor Resection Techniques,” explores the principles, types, and advancements in surgical approaches to tumor removal. It emphasizes the importance of achieving negative margins (R0 resection) to reduce the risk of recurrence while preserving organ function.
Key resection techniques include wide local excision, segmental resection, and en bloc resection, tailored to the size, location, and invasiveness of the tumor. The presentation also highlights minimally invasive methods, such as robotic-assisted and laparoscopic surgeries, which reduce recovery times and improve precision. Emerging innovations like fluorescence-guided surgery and intraoperative imaging are transforming the way surgeons assess margins and ensure complete tumor removal.
The role of multidisciplinary teams, combining surgical expertise with oncology, radiology, and pathology, is also emphasized. This collaborative approach improves patient outcomes and integrates advanced therapies into cancer care.
This resource is invaluable for healthcare professionals, students, and researchers aiming to stay updated on the latest surgical oncology practices. By leveraging technology and innovative techniques, the field of surgical oncology continues to push boundaries, ensuring better care and improved outcomes for cancer patients.

Tumor Resection Techniques Principles, Approaches, and Advances in Surgical Oncology Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

What is Tumor Resection? Surgical removal of a tumor and surrounding tissue to achieve negative margins. An essential component of curative and palliative cancer treatment. Techniques evolve with advances in imaging, surgical tools, and tumor biology understanding. Speaker Notes: Tumor resection is foundational in cancer surgery, whether for curative intent or symptom management. Achieving negative margins minimizes recurrence risk. Reference: National Cancer Institute (NCI). (2022). Cancer Surgery Overview.

Principles of Tumor Resection Achieving Negative Margins (R0 Resection): Removing the tumor with no residual microscopic disease. Wide Local Excision: Includes removal of surrounding normal tissue to ensure margins. En Bloc Resection: Removal of the tumor and adjacent structures (e.g., affected organ or blood vessels) in one piece. Lymph Node Resection: Assessment of regional lymph nodes for staging and management. Speaker Notes: A critical principle is ensuring negative margins to prevent local recurrence. In some cases, adjacent tissues, organs, or lymph nodes are removed to achieve this. Reference: Edge, S. B., & Compton, C. C. (2010). AJCC cancer staging manual.

Types of Tumor Resection Techniques Wide Local Excision: Common in soft tissue tumors like melanoma. Segmental or Partial Resection: Removal of part of the affected organ (e.g., partial hepatectomy). En Bloc Resection: For tumors infiltrating multiple structures (e.g., sarcomas). Debulking Surgery: Removing as much of the tumor as possible when complete resection isn't feasible. Speaker Notes: Each technique is chosen based on the tumor's location, size, and invasiveness. Reference: American Cancer Society (ACS). (2021). Types of cancer surgery.

Special Considerations for Resections Location: Tumors near vital organs (e.g., brain, heart) require precision. Size and Stage: Advanced or large tumors may require extensive resection or reconstruction. Margins: Importance of intraoperative margin assessment to ensure tumor-free edges. Vascular Involvement: Requires vascular reconstruction (e.g., vena cava resection). Speaker Notes: Resection near critical structures often involves additional specialists or advanced tools, like intraoperative imaging. Reference: Blumgart, L. H., et al. (2012). Surgery of the Liver, Biliary Tract, and Pancreas.

Advances in Tumor Resection Techniques Minimally Invasive Surgery: Laparoscopic and robotic-assisted surgery. Benefits: Faster recovery, reduced pain, and improved precision. Intraoperative Imaging: MRI, CT, or ultrasound for real-time guidance. Improves margin assessment and surgical accuracy. Fluorescence-Guided Surgery: Uses dyes to visualize tumor margins (e.g., indocyanine green for liver tumors). 3D Printing: Custom models for preoperative planning and complex reconstructions. Speaker Notes: Emerging tools enhance precision and reduce complications. Fluorescence-guided imaging, for example, highlights the tumor to ensure complete resection. Reference: Norton, J. A., et al. (2021). Surgery: Basic Science and Clinical Evidence.

Case Study: Liver Tumor Resection Patient: 65-year-old with hepatocellular carcinoma. Procedure: Partial hepatectomy with intraoperative ultrasound guidance. Outcome: Negative margins achieved; patient recovered with no complications. Speaker Notes: Case studies like this demonstrate how intraoperative tools can guide surgeons in achieving negative margins and preserving liver function. Reference: Bruix, J., Sherman, M. (2011). Management of hepatocellular carcinoma: An update.

Complications and Challenges Common complications: Bleeding, infection, organ dysfunction. Lymphedema in resections involving lymph nodes. Strategies to minimize complications: Enhanced Recovery After Surgery (ERAS) protocols. Careful intraoperative monitoring and preoperative planning. Speaker Notes: While tumor resection can be curative, complications like bleeding or organ dysfunction need to be managed effectively. Reference: Kehlet, H. (2020). Enhanced recovery after surgery protocols.

Role of Multidisciplinary Teams Surgeons work alongside: Medical oncologists for neoadjuvant/adjuvant therapies. Radiation oncologists for preoperative tumor reduction. Pathologists for margin assessment. Reconstructive surgeons for post-resection restoration. Importance of team discussions for complex cases. Speaker Notes: Multidisciplinary teams ensure the best outcomes by integrating surgery with other cancer treatments. Reference: Basch, E., et al. (2021). Integrative oncology and team-based care.

Key Takeaways Surgical resection remains a cornerstone of cancer treatment. Achieving negative margins (R0) is critical for long-term outcomes. Technological advances, like robotic surgery and intraoperative imaging, improve precision and recovery. Multidisciplinary care is essential for optimal patient outcomes. Speaker Notes: The success of tumor resection lies in precise techniques, advanced tools, and a team-based approach to care.
