About This Presentation

Description : Pain and Symptom Management in Pediatric Oncology
Pain and symptom management are critical aspects of pediatric oncology, significantly impacting the quality of life for children with cancer. This presentation, “Pain and Symptom Management in Pediatric Oncology,” explores effective strategies to alleviate suffering and address the physical, emotional, and psychological needs of young patients.
Cancer-related pain in children can stem from tumors, treatments like chemotherapy and radiation, or invasive procedures. Symptoms such as fatigue, nausea, neuropathy, and emotional distress further burden patients. This presentation highlights a holistic approach to care, incorporating both pharmacological treatments (e.g., opioids, non-opioids, and adjuvant therapies) and non-pharmacological methods (e.g., cognitive-behavioral therapy, music therapy, and physical therapy).
A multidisciplinary team approach is emphasized, involving oncologists, palliative care specialists, psychologists, and nurses. Addressing common barriers, such as fear of opioid use and lack of access to care, this presentation advocates for patient-centered solutions.
By prioritizing effective pain and symptom relief, healthcare providers can improve survival outcomes and enhance the well-being of pediatric patients and their families.
Join us to learn evidence-based practices and innovative techniques that ensure compassionate, comprehensive care for children battling cancer.

Pain and Symptom Management in Pediatric Oncology Improving Quality of Life for Children with Cancer Dr. Vineeta Bose Oncologist & Hematologist

Understanding Pain and Symptoms in Pediatric Oncology Pediatric oncology patients experience unique pain and symptoms due to the disease itself or its treatment. Effective management improves quality of life and treatment outcomes. Goal: To address physical, emotional, and psychological needs of children. Speaker Notes: Explain how cancer and its treatment present challenges for children, requiring tailored approaches for pain and symptom relief. Reference: WHO. (2020). Cancer Pain Relief Guidelines for Children.

Causes of Pain in Children with Cancer Disease-related pain: Tumor infiltration of nerves, bones, or soft tissue. Treatment-related pain: Chemotherapy side effects, post-surgical pain, radiation-induced injuries. Procedural pain: Needle sticks, biopsies, lumbar punctures. Psychosocial pain: Anxiety, fear, and depression. Speaker Notes: Pain in pediatric oncology patients may arise from multiple sources simultaneously, requiring a multimodal approach to management. Reference: Blaney, S. M., & Adamson, P. C. (2012). Current Management of Pediatric Cancer Pain. Pediatric Hematology and Oncology.

Symptom Burden in Pediatric Oncology Fatigue and weakness. Nausea and vomiting. Loss of appetite and weight loss. Sleep disturbances. Neuropathic pain. Emotional symptoms: Anxiety, depression, fear. Speaker Notes: Symptoms often overlap and compound each other, requiring a comprehensive management approach to improve the child’s overall well-being. Reference: Wolfe, J., Hammel, J. F., Edwards, K. E., et al. (2008). Symptoms and suffering at the end of life in children with cancer. New England Journal of Medicine.

Pain Management Strategies Pharmacological Treatments: Opioids (e.g., morphine, fentanyl) for moderate-to-severe pain. Non-opioids (e.g., acetaminophen, NSAIDs) for mild pain. Adjuvant medications (e.g., antidepressants, anticonvulsants) for neuropathic pain. Non-Pharmacological Approaches: Cognitive-behavioral therapy (CBT). Distraction techniques (art therapy, music therapy). Physical therapies (massage, physiotherapy). Speaker Notes: Discuss the need for individualized pain management plans, balancing effective relief with minimizing side effects. Reference: World Health Organization. (2018). WHO guidelines on the pharmacological treatment of persisting pain in children with medical illnesses.

Managing Common Symptoms Nausea and Vomiting: Use of antiemetics like ondansetron and metoclopramide. Fatigue: Encourage light physical activity and proper nutrition. Neuropathic Pain: Use of gabapentin or pregabalin. Emotional Symptoms: Psychological counseling, play therapy, and support groups. Speaker Notes: Highlight that symptom management should address both physical and emotional well-being, as emotional distress often exacerbates physical symptoms. Reference: Hinds, P. S., Hockenberry, M., Gattuso, J. S., et al. (2007). Cancer-related fatigue in children and adolescents. Journal of Pediatric Oncology Nursing.

Multidisciplinary Care Involvement of: Oncologists and pediatricians. Nurses and palliative care specialists. Psychologists and social workers. Physical therapists and nutritionists. Collaborative care ensures holistic management of pain and symptoms. Speaker Notes: Emphasize the need for teamwork to address the multifaceted aspects of pain and symptom management. Reference: Wolfe, J., & Grier, H. E. (2012). Multidisciplinary approaches to symptom management. Pediatric Oncology Reports.

Non-Pharmacological Interventions Relaxation techniques (breathing exercises, meditation). Distraction methods (video games, storytelling, music therapy). Family involvement in care routines. Benefits of integrative approaches like acupuncture and aromatherapy. Speaker Notes: These methods complement medical treatments and provide children with coping tools to manage their pain and symptoms. Reference: Kazak, A. E., & Noll, R. B. (2015). Integrative care for pediatric oncology patients: Benefits and applications. Journal of Pediatric Psychology.

Role of Palliative Care Focuses on symptom relief, emotional support, and improving quality of life. Can be provided alongside curative treatments. Involves the child, family, and healthcare team. Speaker Notes: Discuss how palliative care focuses on relieving suffering and supporting families through challenging times, ensuring dignity and comfort for the child. Reference: World Health Organization. (2019). Integrating palliative care into pediatric oncology programs.

Challenges in Pain and Symptom Management Fear of opioid dependence or overuse. Lack of access to pain management specialists in low-resource settings. Limited awareness of non-pharmacological interventions. Communication barriers with young children. Speaker Notes: Highlight the importance of addressing misconceptions about opioid use and investing in training healthcare professionals. Reference: St. Jude Children’s Research Hospital. (2021). Challenges in pediatric cancer pain management.

Key Takeaways Comprehensive pain management improves quality of life for pediatric oncology patients. A combination of pharmacological, non-pharmacological, and multidisciplinary care is essential. Addressing emotional and psychological needs is as important as managing physical pain.
