About This Presentation

Description : Targeted Therapy in Gynecologic Oncology: Precision Medicine in Cancer Care
Targeted therapy has revolutionized the treatment of gynecologic cancers by focusing on cancer-specific molecular pathways, leading to more effective and personalized care. This presentation, “Targeted Therapy in Gynecologic Oncology,” explores how therapies like PARP inhibitors, VEGF inhibitors, and immune checkpoint inhibitors are transforming outcomes for patients with ovarian, cervical, and endometrial cancers.
PARP inhibitors, such as olaparib, target DNA repair mechanisms, making them particularly effective in cancers with BRCA mutations. VEGF inhibitors, like bevacizumab, disrupt tumor angiogenesis, cutting off blood supply to tumors and delaying disease progression. Immune checkpoint inhibitors, such as pembrolizumab, enhance the immune system’s ability to attack cancer cells, showing promising results in advanced and recurrent gynecologic cancers.
The presentation also highlights the critical role of biomarker testing in precision oncology, allowing treatments to be tailored to specific genetic and molecular profiles. Challenges like drug resistance, treatment costs, and access issues are also discussed, along with future advancements in targeted therapy.
By embracing targeted therapy, gynecologic oncology is moving toward a future of personalized care, improved survival rates, and better quality of life for cancer patients.
Take a step into the future of cancer care with this comprehensive exploration of targeted therapies.

Targeted Therapy in Gynecologic Oncology Advancing Cancer Care with Precision Medicine Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

What is Targeted Therapy? Targeted therapy focuses on cancer-specific molecules and pathways, unlike traditional chemotherapy that affects all dividing cells. Precision oncology uses targeted therapies to treat specific genetic mutations, proteins, or cancer-enabling environments. Revolutionizing gynecologic oncology by improving outcomes and minimizing side effects. Speaker Notes: Introduce the concept of targeted therapy and its role in shifting from conventional chemotherapy to precision medicine. Reference: American Cancer Society (ACS). (2021). What is targeted therapy?.

Key Pathways in Gynecologic Cancers Angiogenesis Pathway: VEGF inhibitors (e.g., bevacizumab). DNA Damage Response: PARP inhibitors (e.g., olaparib). Immune Checkpoints: PD-1/PD-L1 inhibitors. Hormone Pathways: Estrogen and progesterone receptor modulators. Reference: Ledermann, J. A., et al. (2014). Olaparib maintenance therapy in BRCA-mutated ovarian cancer. The New England Journal of Medicine, 366(15), 1382-1392.
Slide 4-

PARP Inhibitors in Ovarian Cancer Mechanism: Inhibit PARP enzymes, causing cell death in tumors with BRCA1/BRCA2 mutations. Examples: Olaparib, niraparib, rucaparib. Indications: Maintenance therapy for advanced ovarian cancer. Recurrent BRCA-mutated ovarian cancer. Clinical Data: Improved progression-free survival in BRCA-mutated ovarian cancer. Speaker Notes: Highlight how PARP inhibitors exploit DNA repair vulnerabilities in cancer cells. Reference: Moore, K., et al. (2018). Maintenance Olaparib in patients with newly diagnosed advanced ovarian cancer. NEJM, 379(26), 2495-2505.
Slide 5-

Anti-VEGF Therapy Mechanism: Inhibits angiogenesis (formation of new blood vessels). Example: Bevacizumab (Avastin). Indications: Advanced and recurrent ovarian cancer. Cervical cancer in combination with chemotherapy. Impact: Delays disease progression by disrupting blood supply to tumors. Speaker Notes: Explain the role of angiogenesis inhibitors, particularly bevacizumab, in controlling tumor growth. Reference: Tewari, K. S., et al. (2014). Bevacizumab for advanced cervical cancer. NEJM, 370(8), 734-743.
Slide 6-

Immune Checkpoint Inhibitors Mechanism: Blocks immune checkpoints (PD-1/PD-L1) to enhance the immune system’s ability to attack cancer cells. Examples: Pembrolizumab (Keytruda), dostarlimab. Indications: Microsatellite instability-high (MSI-H) or mismatch repair-deficient (dMMR) tumors. Advanced endometrial cancer and cervical cancer. Clinical Outcomes: Durable responses in some heavily pretreated patients. Reference: Makker, V., et al. (2020). Pembrolizumab in patients with advanced endometrial cancer. Journal of Clinical Oncology, 38(1), 1-10.
Slide 7-

Hormone Receptor Modulation Mechanism: Targets hormone receptors in cancers that are hormone-sensitive. Examples: Tamoxifen for estrogen receptor-positive endometrial cancer. Progesterone therapies (megestrol acetate). Indications: Hormone-sensitive endometrial and ovarian cancers. Speaker Notes: Highlight the role of hormonal therapies in gynecologic cancers, especially for receptor-positive tumors. Reference: Clarke, C. L., & Sutherland, R. L. (1990). Progestin regulation of cellular proliferation. Endocrine Reviews, 11(2), 266-301.

Role of Biomarkers in Targeted Therapy Biomarkers guide therapy choices for precision oncology: BRCA1/BRCA2 mutations for PARP inhibitors. VEGF expression for anti-angiogenic therapy. PD-L1 expression for immune checkpoint inhibitors. Predictive biomarkers improve treatment efficacy and minimize toxicity. Speaker Notes: Discuss the importance of biomarker testing in personalizing treatment plans. Reference: Birkbak, N. J., & McGranahan, N. (2020). Cancer biomarker discovery and validation. Nature Reviews Cancer, 20(9), 563-579.
Slide 9-

Challenges in Targeted Therapy Drug resistance over time (e.g., PARP inhibitor resistance). High treatment costs and accessibility issues. Toxicities associated with targeted agents. Need for accurate biomarker testing and infrastructure. Speaker Notes: Address the practical and clinical challenges in implementing targeted therapy universally. Reference: Matulonis, U. A., et al. (2021). Mechanisms of resistance to PARP inhibitors. Cancer Discovery, 11(8), 1682-1697.

The Future of Targeted Therapy in Gynecologic Oncology Development of new targets: KRAS inhibitors, bispecific antibodies. Combining targeted therapies with immunotherapy. AI and machine learning for personalized treatment plans. Expanding access through cost reduction and global collaborations. Speaker Notes: Discuss the promising advancements on the horizon in targeted therapy research. Reference: Monk, B. J., et al. (2020). Innovations in targeted therapy for gynecologic malignancies. The Lancet Oncology, 21(8), e387-e397.
