About This Presentation

Description : Treatment Modalities in Breast Oncology: Comprehensive Cancer Care
Breast oncology involves a multidisciplinary approach to treating breast cancer, combining various treatment modalities to ensure the best outcomes for patients. This presentation, "Treatment Modalities in Breast Oncology," provides an in-depth overview of surgical, medical, and radiation treatments, emphasizing their role in managing early-stage, locally advanced, and metastatic breast cancer.
The presentation begins by exploring surgical options such as lumpectomy, mastectomy, and breast reconstruction, tailored to the patient’s cancer stage and preferences. It then delves into systemic therapies, including chemotherapy, hormonal therapy, and targeted treatments like HER2 inhibitors (e.g., trastuzumab). Advances in immunotherapy and antibody-drug conjugates are also highlighted.
Radiation therapy's role in local control and preventing recurrence is discussed, along with its various techniques, such as external beam radiation and brachytherapy. The integration of these therapies based on cancer subtypes and molecular profiling is outlined, emphasizing personalized medicine for optimal outcomes.
By covering both traditional and emerging treatments, this presentation provides a comprehensive guide for healthcare professionals to navigate the complexities of breast cancer care. Visual aids and clinical examples enhance understanding, making this resource both informative and impactful.
Explore innovative approaches in breast oncology to improve patient survival and quality of life.

Treatment Modalities in Breast Oncology Comprehensive Approaches to Managing Breast Cancer Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

Understanding Breast Cancer Treatment Breast cancer treatment involves a multidisciplinary approach. Goals: Curative intent, prolongation of survival, and symptom control. Factors influencing treatment: Cancer type and stage. Hormone receptor (ER/PR) and HER2 status. Patient factors (age, comorbidities, preferences). Speaker Notes: Emphasize the need for a tailored approach based on tumor biology, patient characteristics, and shared decision-making. Reference: Siegel, R. L., Miller, K. D., & Jemal, A. (2020). Cancer Statistics, 2020. CA: A Cancer Journal for Clinicians.

Role of Surgery in Breast Cancer Management Breast-Conserving Surgery (BCS): Lumpectomy followed by radiation. Mastectomy: Removal of the entire breast (simple, modified radical). Axillary Surgery: Sentinel lymph node biopsy (SLNB) for staging. Axillary lymph node dissection (ALND) for node-positive cases. Reconstructive Surgery: Implant-based or autologous tissue reconstruction. Speaker Notes: Discuss the importance of balancing oncological outcomes with patient preferences for cosmesis and quality of life. Reference: Fisher, B., et al. (2002). Lumpectomy vs. mastectomy in breast cancer. The New England Journal of Medicine.
Slide 4-

Chemotherapy in Breast Cancer Treatment Goals: Adjuvant: To eliminate micrometastases post-surgery. Neoadjuvant: To shrink tumors before surgery. Common Chemotherapy Regimens: AC-T (doxorubicin, cyclophosphamide, followed by paclitaxel). CMF (cyclophosphamide, methotrexate, fluorouracil). Side Effects: Nausea, hair loss, fatigue, neutropenia. Speaker Notes: Explain how chemotherapy regimens are tailored based on cancer subtype (e.g., HER2-positive, triple-negative). Reference: Citron, M. L., et al. (2003). Dose-dense chemotherapy in breast cancer. Journal of Clinical Oncology.
Slide 5-

Hormonal Therapy for Hormone Receptor-Positive Breast Cancer Mechanism: Blocks estrogen receptor signaling or reduces estrogen levels. Agents: Selective Estrogen Receptor Modulators (SERMs): Tamoxifen. Aromatase Inhibitors (AIs): Anastrozole, Letrozole, Exemestane. Ovarian suppression with GnRH analogs (e.g., goserelin). Indications: ER/PR-positive breast cancer. Side Effects: Hot flashes, osteoporosis, cardiovascular effects. Speaker Notes: Emphasize the long-term nature of hormonal therapy, typically lasting 5–10 years. Reference: Early Breast Cancer Trialists' Collaborative Group (EBCTCG). (2011). Effectiveness of aromatase inhibitors in postmenopausal women. The Lancet.

Targeted Therapy for HER2-Positive and Other Subtypes HER2-Positive Therapy: Trastuzumab (Herceptin) – Blocks HER2 receptor. Pertuzumab – Combined with trastuzumab for advanced disease. T-DM1 (trastuzumab emtansine) – Antibody-drug conjugate. PARP Inhibitors: Olaparib for BRCA-mutated breast cancers. CDK4/6 Inhibitors: Palbociclib, ribociclib for ER-positive, HER2-negative cancers. Speaker Notes: Discuss the revolution of precision medicine in breast oncology with targeted drugs. Reference: Slamon, D., et al. (2001). Trastuzumab for HER2-positive breast cancer. The New England Journal of Medicine.
Slide 7-

Radiation Therapy in Breast Cancer Indications: Post-lumpectomy (mandatory). Post-mastectomy in high-risk patients. Treatment of regional lymph nodes. Techniques: External beam radiation therapy (EBRT). Brachytherapy (internal radiation). Side Effects: Skin changes, fatigue, lymphedema. Speaker Notes: Highlight the importance of precision in radiation delivery to minimize damage to healthy tissue. Reference: Recht, A., et al. (2001). Postmastectomy radiation in breast cancer. Journal of Clinical Oncology.
Slide 8-

Immunotherapy in Breast Cancer Checkpoint Inhibitors: Atezolizumab – Approved for PD-L1 positive triple-negative breast cancer (TNBC). Mechanism: Boosts the immune system to attack cancer cells. Current Research: Expanding immunotherapy use in HER2-positive and HR-positive breast cancer. Speaker Notes: Discuss how immunotherapy has shown promise, especially in aggressive subtypes like TNBC. Reference: Schmid, P., et al. (2018). Atezolizumab in triple-negative breast cancer. The New England Journal of Medicine.
Slide 9-

Advances in Breast Cancer Treatment Antibody-Drug Conjugates (ADCs): Sacituzumab govitecan for metastatic TNBC. Bispecific Antibodies: Targeting multiple receptors simultaneously. Nanotechnology in Drug Delivery: Enhancing drug targeting with nanoparticles. Speaker Notes: Highlight how emerging therapies are improving outcomes for patients with advanced disease. Reference: Bardia, A., et al. (2021). Sacituzumab govitecan in metastatic TNBC. The Lancet Oncology.
Slide 10-

The Importance of a Multidisciplinary Approach Collaboration between surgeons, oncologists, radiation oncologists, and palliative care teams. Patient-centered care: Addressing physical, emotional, and psychological needs. Speaker Notes: Stress the importance of integrating all modalities for optimal patient outcomes. Reference: National Cancer Institute. (2022). Comprehensive cancer care through multidisciplinary teams.
