About This Presentation

Description : Hormonal Therapy in Cancer Treatment: Mechanisms, Applications, and Advances
Hormonal therapy, also known as endocrine therapy, is a critical treatment approach in managing hormone-dependent cancers such as breast, prostate, and endometrial cancers. It works by either blocking the effects of hormones or reducing their production to inhibit cancer growth. This therapy is widely used in various stages of cancer care, including as adjuvant therapy to prevent recurrence, neoadjuvant therapy to shrink tumors before surgery, or palliative therapy for advanced or metastatic cases.
Key types of hormonal therapy include Selective Estrogen Receptor Modulators (e.g., tamoxifen) and aromatase inhibitors (e.g., letrozole) for breast cancer, as well as androgen deprivation therapies (e.g., leuprolide) for prostate cancer. Recent advancements, such as next-generation anti-androgens and combination therapies with targeted agents, have further improved patient outcomes.
While hormonal therapy offers significant benefits, it may also result in side effects such as hot flashes, mood changes, and long-term risks like osteoporosis, which require careful management.
This presentation delves into the mechanisms, clinical applications, and emerging trends in hormonal therapy, offering insights into its role in improving survival and quality of life for cancer patients. Discover how personalized hormonal therapy is shaping the future of cancer care.

Hormonal Therapy Mechanisms, Indications, and Advances in Hormone-Dependent Cancers Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

What is Hormonal/Endocrine Therapy? Hormonal therapy, also called endocrine therapy, uses medications to block or lower hormones that fuel the growth of certain cancers. Targets hormone-dependent cancers such as breast, prostate, and endometrial cancers. Aims to slow cancer progression, reduce recurrence, or prevent cancer in high-risk patients. Speaker Notes: Hormonal therapy is an essential pillar of cancer treatment, working by altering hormone levels to inhibit cancer growth in hormone-sensitive tumors. Reference: Howell, A., Cuzick, J., Baum, M., et al. (2005). The impact of hormonal therapy on survival in breast cancer. Journal of Clinical Oncology, 23(19), 4305-4311.

How Does Hormonal Therapy Work? Hormone Blockers: Prevent hormones from binding to cancer cells (e.g., tamoxifen for estrogen receptors in breast cancer). Hormone Suppressors: Reduce hormone production by targeting glands (e.g., aromatase inhibitors in postmenopausal women). Mechanism depends on the type of cancer and hormone receptor status. Speaker Notes: Hormonal therapy can either block the action of hormones on cancer cells or reduce their production altogether, targeting the fuel that promotes cancer cell growth. Types of Hormonal Therapy For Breast Cancer: Selective Estrogen Receptor Modulators (SERMs): Tamoxifen. Aromatase Inhibitors: Letrozole, Anastrozole. For Prostate Cancer: Androgen Deprivation Therapy (ADT): GnRH agonists (Leuprolide) and antagonists. Anti-Androgens: Enzalutamide, Bicalutamide. For Endometrial Cancer: Progestins: Medroxyprogesterone acetate. Speaker Notes: Different cancers require tailored hormonal therapies depending on the hormones involved. Breast and prostate cancers have well-established endocrine treatment pathways. Reference: Smith, M. R., Saad, F., Egerdie, B., et al. (2008). When is Hormonal Therapy Used? Adjuvant Therapy: After primary treatment to reduce recurrence risk. Neoadjuvant Therapy: Before surgery to shrink tumors. Palliative Therapy: For advanced-stage or metastatic cancers to slow progression. Preventive Therapy: For individuals at high risk of developing hormone-dependent cancers (e.g., BRCA mutation carriers). Speaker Notes: Hormonal therapy plays versatile roles in cancer care, from curative approaches to managing advanced disease. Reference: Early Breast Cancer Trialists’ Collaborative Group (EBCTCG). (2011). Aromatase inhibitors versus tamoxifen in early breast cancer. Lancet, 377(9783), 1341-1352.

Benefits of Hormonal Therapy Improves survival rates in hormone receptor-positive cancers. Reduces risk of recurrence. Effective for managing symptoms in advanced-stage cancer. Minimal invasiveness compared to surgical options. Speaker Notes: Hormonal therapy has revolutionized outcomes for hormone-receptor-positive cancers, offering significant survival and quality-of-life benefits. Reference: Goss, P. E., & Ingle, J. N. (2009). Aromatase inhibitors in the adjuvant setting. Nature Reviews Clinical Oncology, 6(3), 130-142.

Side Effects of Hormonal Therapy Common side effects: Hot flashes, night sweats. Weight gain. Mood swings, depression. Loss of libido (in men undergoing ADT). Long-term risks: Osteoporosis. Increased risk of cardiovascular disease. Risk of endometrial cancer (tamoxifen). Speaker Notes: While hormonal therapy offers significant benefits, it’s important to monitor and manage its side effects, especially for long-term use. Reference: Labrie, F. (2004). Androgen blockade for prostate cancer. Nature Reviews Urology, 1(1), 21-29.

Advances in Hormonal Therapy Next-generation anti-androgens: Enzalutamide, Apalutamide. Combination therapies with targeted agents (e.g., CDK4/6 inhibitors + endocrine therapy in breast cancer). Precision medicine: Use of biomarkers for personalized hormonal therapy. Speaker Notes: Advances in hormonal therapy include targeted and combination approaches to overcome resistance and improve efficacy. Reference: Turner, N. C., & Slamon, D. J. (2016). CDK4/6 inhibitors in hormone receptor-positive breast cancer. New England Journal of Medicine, 375(20), 1920-1931.

Case Study: Hormonal Therapy for Breast Cancer Patient: 50-year-old postmenopausal woman with hormone receptor-positive breast cancer. Treatment: Aromatase inhibitor (Letrozole) as adjuvant therapy. Outcome: Reduced recurrence risk and well-managed side effects with bone density monitoring. Speaker Notes: This case illustrates the effectiveness of hormonal therapy when personalized to the patient’s profile and monitored for side effects.

Key Takeaways Hormonal therapy is a cornerstone for treating hormone-dependent cancers. Personalized approaches and managing side effects enhance effectiveness. Advances in hormonal therapy continue to improve outcomes for cancer patients. Speaker Notes: Hormonal therapy remains a powerful tool in oncology, with ongoing innovations offering hope for even better patient outcomes. Reference: De Bono, J. S., Logothetis, C. J., Molina, A., et al. (2011). Abiraterone and increased survival in metastatic prostate cancer. New England Journal of Medicine, 364(21), 1995-2005.
