About This Presentation

Description : Surgical Interventions in Reproductive Oncology: A Guide to Techniques and Innovations
Surgical intervention remains one of the most critical approaches in the management of reproductive cancers. This presentation, "Surgical Interventions in Reproductive Oncology," delves into the various surgical techniques used to treat cancers affecting the reproductive system, including cervical, ovarian, endometrial, and testicular cancers.
Key topics include open surgery, minimally invasive techniques such as laparoscopy and robotic-assisted surgery, and fertility-preserving procedures like trachelectomy and ovarian transposition. Cytoreductive surgery for advanced ovarian cancer, sentinel lymph node mapping in endometrial cancer, and orchiectomy for testicular cancer are also highlighted. Advances like Hyperthermic Intraperitoneal Chemotherapy (HIPEC) and robotic-assisted precision surgery have significantly enhanced patient outcomes and recovery times.
In addition, this presentation addresses postoperative care, managing complications, and the psychosocial support needed for cancer patients during recovery. Emphasis is placed on the role of fertility preservation for younger patients and the integration of multidisciplinary teams to optimize care.
This comprehensive guide offers valuable insights for healthcare professionals, medical students, and researchers, providing the knowledge needed to advance surgical approaches in reproductive oncology. Learn how innovation, precision, and patient-centered care are transforming cancer treatment today.

Surgical Interventions in Reproductive Oncology A Comprehensive Guide to Surgical Management of Reproductive Cancers Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

The Role of Surgery in Reproductive Oncology Surgery is a cornerstone in the treatment of reproductive cancers. Goals: Curative intent: Complete removal of the tumor. Palliation: Relieve symptoms and improve quality of life. Staging: Determine the extent of cancer spread. Common cancers requiring surgery: Cervical, ovarian, endometrial, and testicular cancers. Reference: National Cancer Institute (NCI). (2020). Role of surgery in cancer treatment.
Slide 3-

Surgical Techniques in Reproductive Oncology Open Surgery: Traditional approach for advanced or complex cases. Minimally Invasive Surgery (MIS): Laparoscopy and robotic-assisted surgery. Reduced recovery time and complications. Fertility-Sparing Surgery: For younger patients desiring to preserve fertility. Cytoreductive Surgery: Removing as much tumor mass as possible in advanced-stage cancers. Reference: Chan, J. K., & Urban, R. (2019). Minimally invasive surgery in gynecologic oncology. Obstetrics & Gynecology.

Surgical Interventions for Cervical Cancer Early-stage cancer: Cone biopsy (for CIN or early invasive cancer). Simple hysterectomy (removal of the uterus). Locally advanced cancer: Radical hysterectomy with pelvic lymphadenectomy. Fertility-sparing surgery: Radical trachelectomy (removal of the cervix while preserving the uterus). Speaker Notes: Radical hysterectomy is the most common curative surgery for locally advanced cervical cancer. Trachelectomy is an innovative approach for young women. Reference: Bhatla, N., et al. (2021). Revised FIGO staging for cervical cancer 2018. International Journal of Gynecology & Obstetrics.

Ovarian Cancer Surgery Primary treatment for most ovarian cancers. Cytoreductive (Debulking) Surgery: Removal of the uterus, ovaries, fallopian tubes, omentum, and visible tumor masses. Staging surgery: Biopsies, lymph node removal, and peritoneal washings. Interval debulking surgery (after neoadjuvant chemotherapy). Speaker Notes: Cytoreductive surgery aims to remove as much tumor mass as possible, as residual tumor size significantly affects prognosis. Reference: Chi, D. S., et al. (2009). Guidelines for ovarian cancer debulking surgery. Nature Reviews Clinical Oncology.
Slide 6-

Endometrial Cancer Surgery Standard procedure: Total hysterectomy with bilateral salpingo-oophorectomy (THBSO). Sentinel lymph node mapping to assess cancer spread. Minimally invasive surgery (laparoscopy/robotics) as the preferred approach. Fertility-sparing options: Hormonal therapy and hysteroscopic resection in select cases. Speaker Notes: The standard of care is complete removal of the uterus and ovaries. Sentinel lymph node biopsy reduces unnecessary extensive lymph node removal. Reference: Colombo, N., et al. (2016). ESMO guidelines for endometrial cancer treatment.
Slide 7-

Surgical Interventions for Testicular Cancer Standard procedure: Radical inguinal orchiectomy (removal of the affected testicle). Retroperitoneal lymph node dissection (RPLND) for staging or residual disease. Fertility considerations: Sperm banking prior to surgery. Speaker Notes: Orchiectomy is curative for early-stage disease, while RPLND is essential for advanced disease to assess spread. Reference: National Comprehensive Cancer Network (NCCN). (2021). Testicular cancer treatment guidelines.

Fertility-Sparing Surgeries Trachelectomy for early-stage cervical cancer. Ovarian cystectomy for benign ovarian masses. Oophoropexy (ovarian transposition) to preserve ovaries during pelvic radiation. Speaker Notes: Fertility-sparing surgeries are increasingly important for younger cancer patients. These approaches balance oncological safety with the desire for future fertility. Reference: Oktay, K., et al. (2018). Fertility preservation strategies in reproductive oncology.
Slide 9-

Innovations in Reproductive Oncology Surgery Robotic-assisted surgery for precision and minimal invasiveness. HIPEC (Hyperthermic Intraperitoneal Chemotherapy) combined with cytoreductive surgery for advanced ovarian cancer. 3D imaging and navigation for surgical planning. Reference: Fagotti, A., et al. (2016). Role of HIPEC in ovarian cancer. Gynecologic Oncology.
Slide 10-

Complications and Postoperative Care Common complications: Infection, bleeding, and lymphatic fluid accumulation (lymphocele). Rehabilitation: Physical therapy for pelvic floor recovery. Emotional support and counseling. Long-term follow-up: Monitoring for recurrence or secondary cancers. Reference: Lee, Y. Y., & Kim, T. J. (2015). Postoperative management in gynecologic oncology.

Key Takeways Surgical interventions are pivotal in managing reproductive cancers. Advances in minimally invasive techniques and fertility preservation have transformed patient outcomes. Multidisciplinary care is essential for optimal outcomes. Speaker Notes: Emphasize that surgery in reproductive oncology is highly individualized based on cancer type, stage, and patient goals.
