About This Presentation

Description : Systemic Therapies in Reproductive Cancers: Comprehensive Approaches
Systemic therapies play a critical role in treating reproductive cancers by targeting cancer cells throughout the body. This presentation, “Systemic Therapies in Reproductive Cancers,” explores the four main types of systemic treatments: chemotherapy, targeted therapy, immunotherapy, and hormonal therapy. These therapies are crucial for managing advanced and metastatic cancers, including ovarian, cervical, endometrial, prostate, and testicular cancers.
The presentation begins with an overview of chemotherapy, highlighting its mechanism of action and commonly used regimens. It then delves into targeted therapies, such as PARP inhibitors and anti-angiogenic agents, which offer precision treatment by focusing on specific cancer pathways. Immunotherapy, including checkpoint inhibitors like pembrolizumab, represents a groundbreaking approach to harnessing the immune system. Hormonal therapies, such as androgen deprivation therapy for prostate cancer and progestins for endometrial cancer, are also discussed in detail.
Attendees will learn about the benefits of combination therapies, strategies for managing treatment side effects, and advancements in precision oncology, including genomic testing and nanotechnology. With insights from cutting-edge research, this presentation empowers healthcare professionals to optimize systemic therapies for better patient outcomes.
Discover the latest advancements in systemic therapies and their transformative role in reproductive cancer care.

Systemic Therapies in Reproductive Cancers Comprehensive Overview of Chemotherapy, Targeted Therapy, Immunotherapy, and Hormonal Therapy Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

Understanding Systemic Therapies in Reproductive Cancers Systemic therapies involve drugs that travel through the bloodstream to target cancer cells throughout the body. Key types: Chemotherapy, targeted therapy, immunotherapy, and hormonal therapy. Vital in managing advanced or metastatic reproductive cancers (e.g., ovarian, cervical, uterine, prostate, and testicular cancers). Speaker Notes: Highlight the role of systemic therapies in complementing surgery and radiation to improve patient outcomes, especially in advanced-stage cancers. Reference: National Cancer Institute (NCI). (2021). Cancer treatment options overview.
Slide 3-

Chemotherapy in Reproductive Cancers Mechanism: Kills rapidly dividing cancer cells. Common chemotherapy regimens: Ovarian Cancer: Carboplatin + Paclitaxel. Cervical Cancer: Cisplatin-based regimens. Testicular Cancer: BEP (Bleomycin, Etoposide, Cisplatin). Adjuvant, neoadjuvant, or palliative use. Side effects: Nausea, fatigue, hair loss, myelosuppression. Speaker Notes: Chemotherapy is often the backbone of systemic therapy, particularly in ovarian cancer, which has high chemosensitivity. Reference: Siegel, R. L., et al. (2020). Cancer statistics. CA: A Cancer Journal for Clinicians.
Slide 4-

Targeted Therapy in Reproductive Cancers Mechanism: Focuses on specific molecules and pathways driving cancer growth. Key targeted therapies: PARP Inhibitors: Olaparib, niraparib for BRCA-mutated ovarian cancer. Bevacizumab (Avastin): Anti-angiogenesis for ovarian cancer. EGFR Inhibitors: Investigational for cervical cancer. Benefits: Precision treatment with reduced toxicity compared to chemotherapy. Speaker Notes: Targeted therapies represent a shift toward precision oncology, tailoring treatment to tumor biology. Reference: Moore, K., et al. (2018). Maintenance olaparib in patients with newly diagnosed advanced ovarian cancer. NEJM.

Immunotherapy in Reproductive Cancers Checkpoint Inhibitors: Pembrolizumab (Keytruda): Approved for MSI-H or PD-L1 positive cervical cancer. CAR-T Cell Therapy: Emerging treatment for select reproductive cancers. Vaccines: Investigational vaccines targeting HPV and tumor antigens. Benefits: Harnesses the immune system to combat cancer. Speaker Notes: Immunotherapy is gaining traction in reproductive cancers, particularly cervical cancer, due to its ability to target PD-L1 expressing tumors. Reference: Chung, H. C., et al. (2019). Pembrolizumab for previously treated advanced cervical cancer. Journal of Clinical Oncology.
Slide 6-

Hormonal Therapy in Reproductive Cancers Mechanism: Blocks or modifies hormone activity in hormone-sensitive cancers. Key indications: Endometrial Cancer: Progestins (medroxyprogesterone). Prostate Cancer: Androgen deprivation therapy (GnRH agonists, antiandrogens). Ovarian Cancer: Tamoxifen for hormone receptor-positive cancers. Benefits: Non-invasive treatment option with fewer systemic side effects. Speaker Notes: Hormonal therapy is particularly effective in hormone-driven cancers such as prostate and endometrial cancers. Reference: American Cancer Society. (2021). Hormonal therapy for reproductive cancers.

Combining Systemic Therapies for Optimal Outcomes Combination of chemotherapy and targeted therapy (e.g., carboplatin + bevacizumab in ovarian cancer). Sequential use of hormonal therapy and immunotherapy in advanced prostate cancer. Ongoing research into synergistic effects. Speaker Notes: Combination therapies are designed to maximize efficacy while minimizing resistance, especially in advanced-stage or recurrent cancers. Reference: Ledermann, J., et al. (2018). Bevacizumab in ovarian cancer. The Lancet Oncology.

Side Effect Management Common side effects: Chemotherapy: Nausea, neutropenia, neuropathy. Immunotherapy: Immune-related adverse events (e.g., colitis, pneumonitis). Hormonal therapy: Hot flashes, bone loss. Strategies for symptom management: Anti-emetics, growth factors, lifestyle modifications. Speaker Notes: Effective side effect management improves treatment adherence and patient quality of life. Reference: National Comprehensive Cancer Network (NCCN). (2021). Supportive care guidelines.

Advances and Future Directions Precision oncology: Use of genomic testing to guide systemic therapies. Nanoparticle-based drug delivery systems. Bispecific antibodies targeting multiple cancer pathways. Clinical trials investigating combination regimens. Speaker Notes: The future of systemic therapy lies in personalized medicine, leveraging genomics and advanced drug delivery technologies. Reference: Baselga, J., et al. (2020). Advances in systemic therapy for gynecologic cancers. Nature Reviews Clinical Oncology.
