Importance of Gender-Inclusive Reproductive Health Gender identity significantly impacts sexual and reproductive health needs. Inclusive and affirming care improves health outcomes for gender-diverse individuals. Understanding the unique needs of transgender, non-binary, and gender-nonconforming individuals. Speaker Notes: It is essential to recognize that individuals of diverse gender identities have unique reproductive health needs that are often overlooked in traditional gynecological care. Creating an inclusive environment can help reduce healthcare disparities. Reference: Reisner, S. L., Poteat, T., Keatley, J., Cabral, M., Mothopeng, T., Dunham, E., ... & Baral, S. D. (2016). What is Gender Identity? Gender identity: An individual’s internal sense of their own gender (male, female, both, neither). Gender identity differs from sex assigned at birth. Key terms: transgender, non-binary, gender-nonconforming, cisgender. Speaker Notes: Gender identity is a deeply-held sense of self that may or may not align with a person’s sex assigned at birth. Terms like transgender, non-binary, and gender-nonconforming describe various identities within the gender spectrum. Reference: American Psychological Association. (2015). Guidelines for psychological practice with transgender and gender nonconforming people. American Psychologist, 70(9), 832.

Unique Reproductive Health Needs of Transgender and Non-Binary Individuals Reproductive health concerns may include fertility preservation, contraception, menstrual suppression, and cancer screening. Hormone therapy and gender-affirming surgery impact reproductive health. Inclusive counseling for reproductive options is essential. Speaker Notes: Transgender and non-binary individuals may have specific reproductive health needs, such as fertility preservation prior to hormone therapy or contraception options that align with their gender identity. Healthcare providers need to offer supportive and affirming guidance. Reference: Light, A., Wang, L. F., Zeymo, A., & Gomez-Lobo, V. (2018). Family planning and contraception use in transgender men. Contraception, 98(4), 266-269.

Hormone Therapy and Reproductive Health Estrogen therapy for transgender women and testosterone therapy for transgender men. Effects on fertility and reproductive organs. Importance of discussing fertility preservation before initiating hormone therapy. Speaker Notes: Hormone therapy can affect reproductive organs and fertility. Estrogen therapy in transgender women may affect sperm production, while testosterone therapy in transgender men can impact ovulation and menstrual cycles. Counseling on fertility preservation is crucial for those considering future parenthood. Reference: Wierckx, K., Van Caenegem, E., Pennings, G., Elaut, E., Dedecker, D., Van de Peer, F., ... & T’Sjoen, G. (2012). Reproductive wish in transsexual men. Human Reproduction, 27(2), 483-487.

Cancer Screening for Gender-Diverse Individuals Cervical cancer screening for transgender men with intact cervix. Breast cancer screening for transgender women on estrogen therapy. Importance of gender-affirming language and inclusive screening practices. Speaker Notes: Cancer screening recommendations need to be tailored for transgender and non-binary individuals. For example, transgender men with a cervix require regular Pap smears, and transgender women on estrogen should follow breast cancer screening guidelines. Reference: Peitzmeier, S. M., Khullar, K., Potter, J., & Reisner, S. L. (2014). Pap test use is lower among female-to-male patients than non-transgender women. American Journal of Preventive Medicine, 47(6), 808-812.

Fertility Preservation and Family Planning Options for fertility preservation before hormone therapy or surgery. Counseling on family-building options (e.g., sperm banking, egg freezing). Inclusive support for transgender and non-binary individuals who desire to become parents. Speaker Notes: Many transgender and non-binary individuals wish to preserve their fertility before undergoing hormone therapy or surgery. Options such as sperm banking and egg freezing provide opportunities for future family-building. Reference: De Sutter, P., Kira, K., Verschoor, A., & Hotimsky, A. (2002). Creating an Inclusive and Affirming Environment Use gender-affirming language and pronouns. Display inclusive materials and signage. Ensure confidentiality and respect for each patient’s gender identity. Speaker Notes: Creating an inclusive environment starts with using the correct names and pronouns and ensuring privacy. Simple gestures, like having inclusive signage and brochures, can make a huge difference in how comfortable a patient feels. Reference: The Fenway Institute. (2015). Counseling and Psychosocial Support Addressing mental health challenges, such as gender dysphoria, anxiety, and depression. Offering resources for support groups and mental health services. Providing gender-affirming counseling for reproductive choices. Speaker Notes: Transgender and non-binary patients often experience unique mental health challenges. Providing access to support groups, mental health services, and gender-affirming counseling is essential for holistic care. Reference: Bockting, W. O., Miner, M. H., Swinburne Romine, R. E., Hamilton, A., & Coleman, E. (2013). Stigma, mental health, and resilience in an online sample of the US transgender population. American Journal of Public Health, 103(5), 943-951.

Overcoming Barriers to Care Lack of training among healthcare providers. Socioeconomic and insurance challenges. Discrimination and stigma in healthcare settings. Speaker Notes: Many gender-diverse individuals face barriers in accessing healthcare due to discrimination, lack of provider knowledge, and financial constraints. Addressing these barriers requires systemic changes and improved education for healthcare providers. Reference: Grant, J. M., Mottet, L. A., Tanis, J., Harrison, J., Herman, J. L., & Keisling, M.
