MANAGEMENT OF PRETERM LABOR EXT.
Slide 2-

Preterm (Premature) Labor and birth. 2015 Up to date:Incidence and mortality of the preterm infant [Internet]. 2016

Slide 4-

Incidence Preterm birth occurs in 7-12% of all deliveries. Europe : 5–9% USA : 12–13% India : 6-10%
Slide 5-

Risk factor of Preterm birth Maternal age (<18 or >40) Low socioeconomic status Uterine anomaly Depression Previous preterm delivery Inadequate prenatal care Poor nutrition and low BMI Multiple gestation Smoking STD History of second-trimester abortion
Slide 6-

ภาวะแทรกซ้อนและผลที่ตามมาของทารกคลอดก่อนกำหนด Respiratory system : Respiratory distress syndrome (RDS), Bronchopulmonary dysplasia (BPD), Pneumothorax, Pneumonia Sepsis Cardiovascular system: Patent ductus ateriosus (PDA) GI system: Necrotizing enterocolitis (NEC) Neurodevelopment sequelae : cerebral palsy, mental retardation, sensory impairment (visual and auditory deficit), Intraventricular hemorrhage (IVH), Periventricular hemorrhage Behavioural sequelae : dysfunction in cognitive area and emotional control, hyperactivity disorder

Diagnosis of Preterm labor Symptoms : regular contraction & cervical change Cervical change : cervical dilatation >= 3 cm Vaginal Cervical length Short cervical length is associated with an increased risk of PTB Fetal fibronectin glycoprotein secreted by the chorionic tissue. acts as a biological glue which binds blastocyst to endometrium. normally present in cervicovaginal secretions upto 20-22 wks Elevated levels of fFN (>50 ng/mL) after 22 weeks gestation-increased risk of PTB
Slide 8-

Table. Cervical length and risk of preterm birth Recommendation : Recommend TVCL measurement to women with identified or suspected preterm labor (where available) Consider therapeutic interventions when the TVCL is measured at < 25 mm Queensland Clinical Guideline: Preterm labor and birth
Slide 9-

Fetal fibronectin testing Sample : from the posterior fornix of the vagina (cervicovaginal secretions) • Indications: 1- Symptomatic preterm labor 24 - 36 weeks 2- Intact membranes 3- Cervical dilatation <3 cm • Contraindications: 1- Ruptured membranes 2- Vaginal bleeding 3- Cervical cerclage insitu • Relative Contraindications: 1- After the use of lubricants or disinfectants 2- Within 24 hours of coitus or vaginal examination

Treatment of premature labor Inhibition of uterine contractions with tocolysis Corticosteroids to induce fetal lung maturation Treatment of infection with antibiotics Bed rest and hospitalization
Slide 11-

Tocolysis Aim of tocolysis Suppress uterine contractions and delay preterm delivery to : allow intrauterine transfer to an appropriate level facility allow for the administration of corticosteroids. (King .,et al.2003)

Slide 17-

Antenatal corticosteroid การให้สเตียรอยด์ในสตรีที่มีอาการเจ็บครรภ์คลอดก่อนกำหนด จะช่วยลดอัตราการตายของทารกแรก คลอดและภาวะ respiratory distress syndrome (RDS)ลดลง และช่วยลดภาวะ intraventricular hemorrhage The National Institutes of Health (NIH), American College of Obstetricians and Gynecologists (ACOG) และRoyal College of Medicine ต่างก็แนะนำให้ใช้ steroid ในสตรีที่มีอาการเจ็บครรภ์คลอด ก่อน 34 สัปดาห์ (26-32 wks) ชนิดของสเตียรอยด์ มี 2 ชนิด ได้แก่ Betamethasone (12mg; 2times q 24) Dexamethasone (6mg; 4times q 12)
Slide 18-

Antibiotics Routine administration of antibiotics in premature labor without premature rupture of the membranes is not recommended, because the rate of maternal infection is lower ,nor reduction of the neonatal complications. (Subramaniam et al.,2012) Use for PPROM (24-32 wks) and GBS carrier status (Prevention of perinatal group B streptococcal disease —revised guidelines from CDC, 2010)
Slide 19-

Recommended management of preterm labor 1.

Slide 21-

Take home message There is no indication in routine clinical practice for continuing tocolytic therapy for more than 48 hours. Except in some cases (e.g., placenta previa hemorrhage, amniotic sac prolapse). (Schleußner.2013) Using multiple tocolytic drugs associated with a higher risk of adverse effects and should be avoided. (De Heus et al.,2009) There is no evidence that bed rest actually lowers the rate of preterm birth. (Crowther and Han. 2012)
