What is Menstrual Health? Menstrual health encompasses the physical, psychological, and social aspects of menstruation. It includes regular menstrual cycles and freedom from symptoms that impact quality of life. Critical component of women’s reproductive health. Speaker Notes: Menstrual health is essential for the overall well-being of women and includes not only physical health but also psychological and social aspects. It’s important to understand normal menstruation as a baseline to identify any disorders. Reference: Hennegan, J., Shannon, A. K., Rubli, J., Schwab, K. J., & Melendez-Torres, G. J. (2019). Women’s and girls’ experiences of menstruation in low- and middle-income countries: A systematic review and meta-ethnography. PLOS Medicine, 16(5), e1002803.

Understanding the Menstrual Cycle Menstrual Phase: Shedding of the uterine lining. Follicular Phase: Maturation of follicles in the ovaries. Ovulation: Release of an egg from the ovary. Luteal Phase: Uterine lining thickens in preparation for potential pregnancy. Speaker Notes: The menstrual cycle is typically 28 days long, but variations are common. It is regulated by a complex interaction of hormones such as estrogen, progesterone, FSH, and LH. Reference: Fritz, M. A., & Speroff, L. (2010). Clinical Gynecologic Endocrinology and Infertility. Lippincott Williams & Wilkins.

Menstrual Disorders Overview Amenorrhea: Absence of menstruation. Dysmenorrhea: Painful menstruation. Menorrhagia: Heavy menstrual bleeding. Premenstrual Syndrome (PMS): Physical and emotional symptoms before menstruation. Premenstrual Dysphoric Disorder (PMDD): Severe form of PMS. Speaker Notes: Menstrual disorders can significantly affect a woman’s quality of life. They range from irregular or absent periods to heavy and painful menstruation, as well as emotional disturbances. Reference: ACOG. (2018). Dysmenorrhea and other menstrual cycle disorders. Obstetrics & Gynecology, 131(6), e215-e232.

Amenorrhea Primary Amenorrhea: No menstruation by age 15. Secondary Amenorrhea: Absence of menstruation for three or more months in a woman who previously had regular cycles. Causes: Hormonal imbalances, excessive exercise, eating disorders, stress, polycystic ovary syndrome (PCOS). Speaker Notes: Amenorrhea can be primary or secondary. Primary amenorrhea is often due to genetic or anatomical factors, while secondary amenorrhea can be related to lifestyle or underlying medical conditions. Reference: Klein, D. A., & Poth, M. A. (2013). Amenorrhea: An approach to diagnosis and management. American Family Physician, 87(11), 781-788.
Slide 7-

Dysmenorrhea rimary Dysmenorrhea: Pain without an underlying health condition, usually starting in adolescence. Secondary Dysmenorrhea: Pain due to conditions like endometriosis or fibroids. Management: NSAIDs, hormonal birth control, lifestyle changes, physical therapy. Speaker Notes: Dysmenorrhea, or painful periods, is one of the most common menstrual complaints. Primary dysmenorrhea often improves with age, while secondary dysmenorrhea may need further medical intervention. Reference: Dawood, M. Y. (2006). Primary dysmenorrhea: Advances in pathogenesis and management. Obstetrics & Gynecology, 108(2), 428-441.

Menorrhagia Defined as bleeding that lasts more than 7 days or excessive bleeding that disrupts daily activities. Causes: Hormonal imbalance, uterine fibroids, polyps, clotting disorders. Treatment Options: Hormonal therapies, antifibrinolytic medications, surgery (e.g., endometrial ablation). Speaker Notes: Menorrhagia is not only physically debilitating but can also lead to anemia. Treatment depends on the underlying cause and severity of the bleeding. Reference: National Institute for Health and Care Excellence (NICE). (2018). Heavy menstrual bleeding: Assessment and management.
Slide 9-

Diagnosing Menstrual Disorders Medical history and symptom tracking. Physical and pelvic exams. Diagnostic tests: Blood tests, ultrasounds, hysteroscopy, laparoscopy. Speaker Notes: Diagnosis involves understanding the patient's medical history, performing physical examinations, and using diagnostic tests if necessary. Reference: American College of Obstetricians and Gynecologists (ACOG). (2019). Evaluation of abnormal uterine bleeding in reproductive-aged women.
Slide 10-

Treatment Options for Menstrual Disorders Lifestyle changes (diet, exercise, stress management). Medications (NSAIDs, hormonal treatments, antidepressants for PMS/PMDD). Surgical options (for cases like fibroids or endometriosis). Speaker Notes: Treatment options vary widely and should be tailored to each patient’s symptoms and underlying causes. Lifestyle changes can help, but medications and surgeries may be necessary for more severe conditions. Reference: Lethaby, A., Farquhar, C., & Cooke, I. (2005). Antifibrinolytics for heavy menstrual bleeding. Cochrane Database of Systematic Reviews, (4).

Importance of Education and Awareness Early diagnosis and management improve quality of life. Encouraging open discussions about menstrual health. Reducing stigma associated with menstrual disorders. Speaker Notes: Educating women on menstrual health and reducing stigma around discussing these issues is crucial for early diagnosis and treatment. Reference: Hennegan, J., Shannon, A. K., Rubli, J., Schwab, K. J., & Melendez-Torres, G. J. (2019). Women’s and girls’ experiences of menstruation in low- and middle-income countries.
