What is Gynecologic Endocrinology? The study of hormonal regulation in the female reproductive system. Focuses on the hormones that control menstruation, fertility, pregnancy, and menopause. Intersects with reproductive health, sexual health, and overall endocrine health. Speaker Notes: Gynecologic endocrinology is crucial for understanding how hormones impact various stages of a woman’s life, from puberty to menopause. Kandeel, F. R., & Santoro, N. F. (Eds.). (2019). Endocrinology: Adult and Pediatric. Elsevier.

The Hypothalamic-Pituitary-Ovarian (HPO) Axis The HPO axis regulates the menstrual cycle and reproductive hormones. Hypothalamus: Releases GnRH (Gonadotropin-Releasing Hormone). Pituitary Gland: Releases FSH (Follicle-Stimulating Hormone) and LH (Luteinizing Hormone). Ovaries: Produce estrogen and progesterone. Speaker Notes: The HPO axis is a feedback loop that controls the menstrual cycle and ovulation through a delicate hormonal balance. Reference: Ganong, W. F. (2019). Review of Medical Physiology. McGraw-Hill Education.

Menstrual Cycle Phases Follicular Phase: FSH stimulates follicle development; estrogen levels rise. Ovulation: LH surge triggers ovulation around day 14. Luteal Phase: Corpus luteum forms; progesterone peaks to prepare uterus for pregnancy. Menstruation: Decline in progesterone and estrogen leads to shedding of the endometrial lining. Speaker Notes: Each phase of the menstrual cycle is regulated by a specific set of hormonal changes that prepare the body for potential pregnancy. Reference: Johnson, M. H., & Everitt, B. J. (2013). Essential Reproduction. John Wiley & Sons.

Key Hormones in Gynecologic Endocrinology Estrogen: Responsible for secondary sexual characteristics, endometrial growth. Progesterone: Supports pregnancy, regulates menstrual cycle. FSH and LH: Regulate follicle development and ovulation. Prolactin: Involved in milk production. Androgens: Small amounts in females; influence libido and muscle strength. Speaker Notes: Understanding the role of these key hormones helps in diagnosing and treating various gynecological conditions. Reference: Speroff, L., & Fritz, M. A. (2005). Clinical Gynecologic Endocrinology and Infertility. Lippincott Williams & Wilkins.

Common Endocrine Disorders in Gynecology Polycystic Ovary Syndrome (PCOS): Hormonal imbalance, irregular periods, excess androgen. Hypogonadotropic Hypogonadism: Low FSH/LH, results in amenorrhea. Hyperprolactinemia: Excess prolactin, causes irregular menstruation. Premature Ovarian Insufficiency (POI): Early decline in ovarian function. Speaker Notes: These endocrine disorders affect reproductive health and often require a multidisciplinary approach for management. Reference: Azziz, R., Woods, K. S., Reyna, R., Key, T. J., Knochenhauer, E. S., & Yildiz, B. O. (2004). The prevalence and features of the polycystic ovary syndrome in an unselected population. The Journal of Clinical Endocrinology & Metabolism, 89(6), 2745-2749.
Slide 8-

Polycystic Ovary Syndrome (PCOS) Symptoms: Irregular periods, hirsutism, acne, obesity. Hormonal Imbalance: Increased LH, androgens; decreased FSH. Management: Lifestyle changes, oral contraceptives, anti-androgen medications, insulin sensitizers. Speaker Notes: PCOS is one of the most common endocrine disorders in gynecology, affecting fertility and metabolic health. Reference: Legro, R. S., et al. (2013). Diagnosis and treatment of polycystic ovary syndrome: an Endocrine Society clinical practice guideline. The Journal of Clinical Endocrinology & Metabolism, 98(12), 4565-4592.
Slide 9-

Premature Ovarian Insufficiency (POI) Definition: Decline in ovarian function before age 40. Causes: Genetic factors, autoimmune disorders, chemotherapy. Symptoms: Irregular periods, hot flashes, infertility. Management: Hormone Replacement Therapy (HRT), fertility options. Speaker Notes: POI can have significant psychological impacts on patients due to its effect on fertility and early menopause symptoms. Reference: Torrealday, S., Kodaman, P., & Pal, L. (2017). Premature Ovarian Insufficiency—an update on recent advances in understanding and management. F1000Research, 6.
Slide 10-

Hyperprolactinemia Cause: Elevated prolactin levels, often due to pituitary adenomas. Symptoms: Irregular menstruation, galactorrhea, infertility. Treatment: Dopamine agonists, surgical options for pituitary tumors. Speaker Notes: High prolactin levels disrupt the HPO axis, inhibiting FSH and LH, leading to menstrual irregularities. Reference: Molitch, M. E. (2017). Diagnosis and treatment of pituitary adenomas: a review. JAMA, 317(5), 516-524.
Slide 11-

Menopause and Hormone Replacement Therapy (HRT) Symptoms: Hot flashes, mood changes, vaginal dryness. Hormone Changes: Decreased estrogen and progesterone. HRT Benefits: Alleviates symptoms, prevents osteoporosis. Risks: Increased risk of certain cancers, blood clots. Speaker Notes: HRT is a common treatment for menopause symptoms but requires careful consideration of risks and benefits. Reference: Manson, J. E., & Kaunitz, A. M. (2016). Menopause management—getting clinical care back on track. New England Journal of Medicine, 374(9), 803-806.
Slide 12-

Diagnostic Tests in Gynecologic Endocrinology Blood Tests: Hormone levels (FSH, LH, estrogen, prolactin). Ultrasound: Imaging of ovaries, follicles, endometrium. MRI and CT Scans: For pituitary tumors and reproductive organs. Bone Density Test: For postmenopausal women. Speaker Notes: Diagnostic tests help identify hormonal imbalances, structural abnormalities, and monitor health over time. Reference: Barbieri, R. L. (2014). Pathophysiology of gynecologic disorders. McGraw-Hill Education.
