About This Presentation

Description : Palliative Surgical Interventions: Improving Quality of Life for Cancer Patients
Palliative surgical interventions play a vital role in enhancing the quality of life for patients with advanced or incurable cancer. Unlike curative surgeries, palliative procedures focus on relieving symptoms such as pain, obstruction, or bleeding, thereby improving functionality and providing physical and psychological comfort.
Common indications for palliative surgery include bowel obstructions, malignant effusions, hemorrhage control, and spinal cord compression caused by tumors. Procedures such as colostomies, bypass surgeries, pleurodesis, and tumor debulking aim to alleviate these symptoms, enabling patients to regain independence and dignity.
A multidisciplinary approach involving surgeons, oncologists, and palliative care specialists ensures patient-centered decision-making. The benefits of these interventions must always be weighed against potential risks, such as surgical complications and recovery challenges.
Through real-world case studies, this presentation highlights the importance of aligning surgical goals with the patient’s preferences and quality-of-life expectations. By focusing on symptom relief and holistic care, palliative surgical interventions empower patients to spend meaningful time with loved ones and live more comfortably despite their illness.
Learn more about these life-enhancing surgical options, their indications, and the importance of compassionate, multidisciplinary cancer care in this insightful presentation.

Palliative Surgical Interventions Enhancing Quality of Life in Advanced Cancer Care Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

Understanding Palliative Surgical Interventions Palliative surgeries aim to alleviate symptoms and improve quality of life. Unlike curative surgeries, these procedures are not intended to eradicate the disease. Integral to comprehensive palliative care in oncology. Speaker Notes: Discuss the distinction between palliative and curative surgery, emphasizing the focus on patient comfort and well-being. Reference: Ferrell, B. R., Temel, J. S., Temin, S., et al. (2017). Integration of palliative care into standard oncology care: ASCO clinical practice guideline update. Journal of Clinical Oncology.

Goals of Palliative Surgical Interventions Symptom Relief: Reduce pain, bleeding, obstruction, or pressure. Improved Function: Restore or maintain bodily functions (e.g., bowel movement, breathing). Psychological Relief: Reduce the mental burden of disease-related symptoms. Quality of Life: Enable patients to carry out daily activities and maintain dignity. Speaker Notes: Highlight that the primary goal is to ensure the patient’s comfort and functional independence whenever possible. Reference: Davis, M. P., & Walsh, D. (2010). Surgical palliation in cancer care: A review and update. American Journal of Hospice and Palliative Medicine.

Common Indications for Palliative Surgery Bowel Obstruction: Colorectal cancer, ovarian cancer. Malignant Effusions: Pleural or peritoneal effusion causing dyspnea. Hemorrhage Control: Bleeding tumors (e.g., gastric or cervical cancer). Spinal Cord Compression: Relieve pressure caused by metastatic tumors. Pain Management: Debulking tumors pressing on nerves or organs. Speaker Notes: Each indication addresses specific symptoms to improve physical comfort and functionality. Reference: Ripamonti, C. I., & Fulfaro, F. (2004). Surgical interventions in palliative care: Focus on symptom control and quality of life. Critical Reviews in Oncology/Hematology.

Types of Palliative Surgical Interventions Bypass Surgery: Gastrointestinal bypass for bowel obstructions. Stent placement for biliary or esophageal obstructions. Debulking Surgery: Reducing tumor mass to relieve pressure or pain. Pleurodesis or Catheter Placement: Relieve dyspnea caused by pleural effusion. Ostomies: Colostomy or urostomy to bypass obstructed pathways. Decompression Surgery: Laminectomy for spinal cord compression. Speaker Notes: Explain how each type of surgery aligns with specific symptoms and improves functionality. Reference: Kim, J. H., & D’Amico, T. A. (2017). Role of surgical interventions in palliative care. Journal of Thoracic Disease.

Risks and Challenges of Palliative Surgery Potential Risks: Surgical complications (infection, bleeding). Prolonged recovery time. Impact on already fragile health. Challenges: Determining whether surgery aligns with patient goals. Ethical considerations: “Do no harm.” Multidisciplinary decision-making. Speaker Notes: Highlight the importance of weighing benefits against risks, especially in patients with limited life expectancy. Reference: Clarke, M. G., et al. (2007). Palliative surgery for cancer patients: A systematic review. The Lancet Oncology.

Decision-Making in Palliative Surgery Shared Decision-Making: Align treatment with patient values and goals. Ethical Considerations: Avoid unnecessary or overly invasive procedures. Factors to Consider: Patient’s functional status (e.g., ECOG performance score). Symptom severity and expected benefits. Life expectancy and quality of life. Speaker Notes: Discuss the role of the multidisciplinary team and the importance of patient autonomy. Reference: Bruera, E., et al. (2015). Palliative care in oncology: A patient-centered approach. The Lancet Oncology.

Case Studies Case 1: Bowel obstruction in advanced ovarian cancer managed with a colostomy to relieve symptoms. Case 2: Pleural effusion in metastatic lung cancer treated with a pleurodesis. Case 3: Debulking surgery for spinal cord compression in metastatic breast cancer. Speaker Notes: Provide examples to illustrate the benefits and challenges of palliative surgical interventions.
Slide 9-

Multidisciplinary Approach Collaboration between: Surgeons Oncologists Palliative care specialists Psychologists and social workers Ensures holistic and patient-centered care. Speaker Notes: Stress the need for teamwork in optimizing surgical outcomes and addressing the patient’s overall needs. Reference: Higginson, I. J., & Evans, C. J. (2010). What is the evidence that palliative care teams improve outcomes for cancer patients and their families? Cancer Journal.

Benefits of Palliative Surgical Interventions Immediate symptom relief. Improved functional independence. Enhanced psychological well-being. Enables patients to spend meaningful time with loved ones. Speaker Notes: Highlight real-life benefits such as reduced pain, better mobility, and restored dignity for patients with advanced cancer.

Key Takeaways Palliative surgeries prioritize quality of life, not cure. Decision-making must be patient-centered and multidisciplinary. A careful balance of benefits, risks, and patient goals is crucial.
