About This Presentation

Description : Fertility Preservation in Cancer Patients: Options, Advances, and Hope
Cancer treatments such as chemotherapy, radiation, and surgery often pose risks to fertility, leaving many patients concerned about their reproductive future. This presentation, "Fertility Preservation in Cancer Patients," highlights the available options for safeguarding fertility, empowering patients to make informed decisions during a challenging time.
Learn about techniques like egg freezing, sperm freezing, embryo freezing, and ovarian tissue preservation. We discuss their success rates, advantages, and suitability for patients based on age, cancer type, and treatment plans. For male patients, sperm freezing is the most common and accessible option, while testicular tissue freezing offers hope for prepubescent boys. Female patients may consider egg and embryo freezing or newer experimental methods like ovarian tissue preservation.
Additionally, this presentation emphasizes the role of early counseling, multidisciplinary collaboration, and the importance of discussing fertility preservation before cancer treatment begins. Ethical, financial, and emotional challenges in fertility preservation are also addressed, with insights into advocacy for affordable and accessible services.
By integrating fertility preservation into cancer care, we can improve the quality of life for survivors, offering them the opportunity to build families post-treatment. Let this guide serve as a roadmap to navigating reproductive health during and after cancer.

Fertility Preservation in Cancer Patients Empowering Cancer Patients to Plan for Their Reproductive Future Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

Why Fertility Preservation Matters Advances in cancer treatment have improved survival rates. Many treatments (e.g., chemotherapy, radiation, surgery) can impact fertility. Fertility preservation provides patients with the opportunity to have biological children post-treatment. Speaker Notes: Discuss the growing importance of fertility preservation as survivorship becomes a key focus in oncology care. Reference: Oktay, K., Harvey, B. E., Partridge, A. H., et al. (2018). Fertility Preservation in Patients With Cancer: ASCO Clinical Practice Guideline Update. Journal of Clinical Oncology, 36(19), 1994-2001.

Impact of Cancer Treatments on Fertility Chemotherapy: Damages eggs, sperm, and reproductive tissues (e.g., alkylating agents like cyclophosphamide). Radiation: Impacts ovaries, testes, and surrounding reproductive organs. Surgery: Removal of reproductive organs (e.g., hysterectomy, orchiectomy). Hormonal Therapy: Alters hormonal balance, affecting fertility. Speaker Notes: Highlight specific cancer treatments and their effects on fertility in both men and women. Reference: Wallace, W. H. B., & Anderson, R. A. (2012). Fertility preservation in the male with cancer. Pediatric Blood & Cancer, 60(5), 712-718.

Fertility Preservation Options for Women Egg Freezing (Oocyte Cryopreservation): Freezing unfertilized eggs for future use. Embryo Freezing: Fertilizing eggs with sperm and freezing the embryos. Ovarian Tissue Freezing: Removing and freezing ovarian tissue for transplantation later. Ovarian Suppression: Use of GnRH agonists to protect ovarian function during treatment. Speaker Notes: Explain that the choice of fertility preservation depends on the patient’s age, type of cancer, and treatment timeline. Reference: Practice Committee of the American Society for Reproductive Medicine (2019). Fertility preservation in patients undergoing gonadotoxic therapy or gonadectomy: A committee opinion. Fertility Preservation Options for Men Sperm Freezing (Cryopreservation): Collecting and freezing sperm for future use. Testicular Tissue Freezing: Experimental option for prepubescent boys. Shielding Reproductive Organs: Protecting the testes during radiation therapy. Speaker Notes: Emphasize the ease and effectiveness of sperm cryopreservation, which is the most commonly used method for men. Reference: Agarwal, A., Gupta, R., & Sharma, R. (2005). Cryopreservation of sperm: An overview. Reproductive Biomedicine Online, 10(3), 376-384.

Oncofertility for Children and Adolescents Boys: Testicular tissue freezing (experimental). Girls: Ovarian tissue freezing (experimental) or ovarian suppression. Counseling parents about risks and benefits. Speaker Notes: Discuss how fertility preservation options for prepubescent patients are largely experimental but offer hope for future advances. Reference: Ginsberg, J. P. (2011). Educational paper: The effect of cancer therapy on fertility, the assessment of fertility and fertility preservation options for pediatric patients. Counseling Patients on Fertility Preservation Providing tailored information based on the patient’s age, cancer type, and treatment plan. Discussing timing, success rates, and costs of fertility preservation. Importance of shared decision-making and emotional support. Speaker Notes: Emphasize the need for empathetic communication and multidisciplinary care involving oncologists, reproductive specialists, and counselors. Reference: Quinn, G. P., & Vadaparampil, S. T. (2013). Fertility preservation and adolescent/young adult cancer patients: Physician communication challenges. Journal of Adolescent and Young Adult Oncology, 2(4), 123-125.

Ethical and Financial Considerations Ethical challenges: Preserving fertility vs. delaying cancer treatment. Financial concerns: High costs of fertility preservation procedures. Advocacy for insurance coverage and financial support programs. Speaker Notes: Address systemic barriers and advocate for better access to fertility services. Reference: Ethics Committee of the American Society for Reproductive Medicine (2018). Fertility preservation and reproduction in cancer patients. Fertility and Sterility, 110(3), 378-382.

Conclusion Fertility preservation allows cancer survivors to plan for a family. Early counseling and tailored strategies are critical. Advances in technology offer hope for more inclusive and effective options. Speaker Notes: Summarize the importance of integrating fertility preservation into cancer care to improve patients' quality of life.
