About This Presentation

Description : Obstetrics - Neonatal Care and Resuscitation
Newborn care immediately following birth is critical for ensuring a healthy start to life, especially for infants needing urgent medical intervention. Our presentation on ""Obstetrics - Neonatal Care and Resuscitation"" covers the essential steps healthcare professionals should take to assess, stabilize, and, if necessary, resuscitate newborns.
This comprehensive guide begins with the basics of newborn assessment, including the Apgar score, and then progresses to the structured steps of neonatal resuscitation. Topics include initial stabilization, airway management, positive pressure ventilation (PPV), and chest compressions for infants with low heart rates. The presentation also addresses advanced resuscitation methods such as intubation and medication administration for cases that require further intervention.
In addition to resuscitation techniques, we discuss post-resuscitation care, including monitoring in a neonatal intensive care unit (NICU), to ensure continued support for infants after stabilization.
This presentation is ideal for healthcare professionals, students, and anyone involved in neonatal care who seeks a clear, step-by-step guide on newborn resuscitation and care practices, supported by the latest guidelines and research.

Introduction to Neonatal Care and Resuscitation Neonatal care: Critical for a newborn’s health and survival. Resuscitation may be needed if newborns have breathing or circulation issues at birth. Goal: Ensure newborns receive timely, effective care to transition to extra-uterine life. Speaker Notes: Neonatal care and resuscitation are essential to ensure that newborns, particularly those with complications at birth, receive the best possible start in life. Rapid, effective intervention can significantly impact survival and long-term health. Reference: WHO. (2012). Guidelines on Basic Newborn Resuscitation. World Health Organization.

Immediate Newborn Assessment Apgar Score: Evaluates Appearance, Pulse, Grimace, Activity, and Respiration. Vital signs: Heart rate, respiratory effort, color. Classification: Healthy, in need of observation, or in need of resuscitation. Speaker Notes: The Apgar score is taken at 1 and 5 minutes after birth. It provides a quick assessment of the baby’s health, helping to determine if additional medical care is needed immediately. Reference: Apgar, V. (1953). A proposal for a new method of evaluation of the newborn infant. Current Researches in Anesthesia & Analgesia, 32(4), 260-267.

Essential Neonatal Care Temperature control: Immediate drying, skin-to-skin contact, warming. Clearing airways if needed. Umbilical cord care and cutting. Administration of vitamin K and eye prophylaxis. Early breastfeeding initiation. Speaker Notes: Essential neonatal care involves a series of steps to stabilize the newborn, prevent infection, and ensure proper nutrition. Maintaining body temperature and encouraging early breastfeeding are critical for survival and development. Reference: WHO. (2006). Essential Newborn Care. World Health Organization.

Indications for Neonatal Resuscitation Lack of spontaneous breathing or weak breathing. Heart rate below 100 bpm. Low Apgar score (0-3 at 1 minute). Persistent cyanosis (blue discoloration). Speaker Notes: Neonatal resuscitation is required when there is a lack of adequate breathing or heart rate, which are critical signs that a newborn is struggling to adapt after birth. Reference: American Heart Association. (2020). Neonatal Resuscitation Program Guidelines. American Heart Association.

Neonatal Resuscitation Steps 1. Positioning and Airway Clearing: Place the newborn in a neutral position, clear the airway if needed. 2. Stimulation: Gentle tactile stimulation (e.g., rubbing the back). 3. Positive Pressure Ventilation (PPV): Start if the newborn is not breathing adequately. 4. Chest Compressions: Begin if the heart rate is below 60 bpm after ventilation. Speaker Notes: Basic resuscitation steps are carried out sequentially. It’s essential to have a structured approach and act promptly to stabilize the newborn. Reference: Perlman, J. M., et al. (2015). Neonatal resuscitation: 2015 International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science With Treatment Recommendations. Pediatrics, 136(Supplement 2), S120-S166.

Positive Pressure Ventilation (PPV) Use a bag-mask or T-piece resuscitator. Target: Ventilation rate of 40-60 breaths per minute. Check for chest movement; adjust if necessary. Reassess every 30 seconds. Speaker Notes: Positive pressure ventilation is vital for newborns who are not breathing adequately. Ensuring chest movement is key to delivering effective ventilation. Reference: Wyckoff, M. H., et al. (2015). Neonatal resuscitation guidelines. Circulation, 132(16_suppl_1), S543-S560.
Slide 9-

Chest Compressions Indicated if heart rate <60 bpm despite ventilation. Technique: Two-thumb encircling hands method. Compression-to-ventilation ratio of 3:1. Reassess every 60 seconds. Speaker Notes: If heart rate remains low despite ventilation, chest compressions should be initiated. The two-thumb technique is most effective in newborns. Reference: American Academy of Pediatrics. (2016). Textbook of Neonatal Resuscitation (7th ed.).
Slide 10-

Advanced Resuscitation Techniques Intubation: Required if ventilation is not effective via bag-mask. Medication: Epinephrine administration if no improvement. Umbilical Venous Access: For medication administration. Use of oxygen: Avoid hyperoxia; target oxygen saturation based on age in minutes. Speaker Notes: Advanced resuscitation techniques are used if basic steps are insufficient. Intubation and medication administration may be needed in severe cases. Reference: Saugstad, O. D., & Aune, D. (2011). Optimal oxygenation at birth and in the neonatal period. Pediatric Research, 69, 21-29.
Slide 11-

Post-Resuscitation Care Transfer to NICU if ongoing monitoring is needed. Monitor vital signs and blood glucose. Maintain normal body temperature. Neurological assessment and continued respiratory support. Speaker Notes: After successful resuscitation, the newborn should be carefully monitored in a NICU setting to manage any complications that may arise. Reference: Kamlin, C. O., & Dawson, J. A. (2014). Post-resuscitation care following neonatal resuscitation. Seminars in Fetal and Neonatal Medicine, 19(5), 308-314.

Key Takeaways
Slide 13-

Early assessment and effective resuscitation are crucial for newborn survival. Follow the structured steps: airway, breathing, circulation. Post-resuscitation care is essential to ensure long-term well-being. Speaker Notes: To summarize, neonatal care and resuscitation require prompt action and adherence to established guidelines. Proper post-resuscitation care is crucial for the best outcomes.
