What is Urogynecology? Urogynecology, also known as Female Pelvic Medicine and Reconstructive Surgery (FPMRS), is a subspecialty focused on pelvic floor disorders. Deals with conditions such as pelvic organ prolapse, urinary incontinence, and fecal incontinence. Combines elements of gynecology and urology for comprehensive pelvic care. Speaker Notes: Urogynecology addresses disorders that affect the female pelvic organs, particularly those involving weakened pelvic floor muscles. It's a unique specialty that merges gynecological and urological expertise to provide targeted treatment. Reference: Brubaker, L., & Norton, P. (2013). Female Pelvic Medicine and Reconstructive Surgery. Elsevier.

Anatomy of the Female Pelvic Floor Key structures: Bladder, uterus, rectum, and pelvic floor muscles. Pelvic floor muscles support the organs and maintain continence. Ligaments and connective tissues help stabilize pelvic organs. Speaker Notes: The pelvic floor muscles form a "hammock" that holds the bladder, uterus, and rectum in place. Weakness or damage to these muscles and tissues can lead to various disorders. Reference: Ashton-Miller, J. A., & DeLancey, J. O. (2007). Functional anatomy of the female pelvic floor. Annals of the New York Academy of Sciences, 1101, 266-296.

Common Pelvic Floor Disorders Pelvic Organ Prolapse (POP): Bladder, uterus, or rectum descends into or outside the vaginal canal. Urinary Incontinence (UI): Loss of bladder control, often stress or urge incontinence. Fecal Incontinence (FI): Inability to control bowel movements. Speaker Notes: The most common disorders in urogynecology are pelvic organ prolapse, urinary incontinence, and fecal incontinence. Each of these can severely impact quality of life. Reference: Smith, F. J., et al. (2010). Incidence and lifetime risk of pelvic organ prolapse surgery in women: A cohort study. BJOG, 117(12), 1506-1513.

Pelvic Organ Prolapse (POP) Types of POP: Cystocele (bladder), Rectocele (rectum), Uterine prolapse. Symptoms: Vaginal bulge, pelvic pressure, difficulty with urination or bowel movements. Risk factors: Childbirth, age, obesity, chronic constipation. Speaker Notes: POP occurs when pelvic organs drop due to weak or damaged pelvic floor muscles. It’s more common in women who have had vaginal births and becomes more likely with age. Reference: Olsen, A. L., et al. (1997). Epidemiology of surgically managed pelvic organ prolapse and urinary incontinence. Obstetrics & Gynecology, 89(4), 501-506.
Slide 7-

Urinary Incontinence (UI) Types: Stress incontinence, urge incontinence, mixed incontinence. Symptoms: Leakage during activities, sudden strong urge to urinate. Causes: Weak pelvic muscles, menopause, nerve damage, certain medications. Speaker Notes: UI is one of the most common complaints among women. Stress incontinence typically occurs during activities like sneezing, while urge incontinence involves a sudden, intense need to urinate. Reference: Abrams, P., et al. (2010). 4th International Consultation on Incontinence Recommendations. Neurourology and Urodynamics, 29(1), 213-240.

Diagnostic Tools in Urogynecology Pelvic Exam: Assess strength of pelvic muscles and signs of prolapse. Urodynamic Testing: Measures bladder function and pressure. Imaging: Ultrasound, MRI for detailed visualization of pelvic organs. Speaker Notes: Diagnostic tools help assess the severity and type of pelvic floor disorders, allowing for tailored treatment. Urodynamic testing, in particular, provides insights into bladder function. Reference: Mowat, A., Maher, C., & Baessler, K. (2008). Urodynamics: Why, when and how?. Australian and New Zealand Journal of Obstetrics and Gynaecology, 48(4), 335-341.

Treatment Options Non-Surgical Treatments: Pelvic floor exercises, pessaries, lifestyle changes. Medications: Anticholinergics for overactive bladder, hormone therapy. Surgical Options: Sling procedures, prolapse repair, sacrocolpopexy. Speaker Notes: Treatment options vary based on the disorder’s severity. Non-surgical methods are often the first line of defense, but surgery may be recommended for more severe cases. Reference: Brubaker, L., & Moalli, P. A. (2006). Surgical treatment of stress urinary incontinence and pelvic organ prolapse. Obstetrics & Gynecology, 107(1), 151-168.
Slide 10-

Pelvic Floor Physical Therapy Exercises to strengthen pelvic muscles. Biofeedback and electrical stimulation for muscle awareness. Behavioral therapy for bladder training. Speaker Notes: Pelvic floor physical therapy is a cornerstone of conservative management for pelvic floor disorders. It can help women strengthen their muscles and improve symptoms. Reference: Dumoulin, C., & Hay-Smith, E. J. (2010). Pelvic floor muscle training versus no treatment, or inactive control treatments, for urinary incontinence in women. Cochrane Database of Systematic Reviews.
Slide 11-

Surgical Approaches Sling Procedures: To treat stress urinary incontinence. Sacrocolpopexy: Surgical procedure for prolapse using mesh support. Colpocleisis: Vaginal closure procedure for severe prolapse in older women. Speaker Notes: Surgical interventions may involve procedures like sling placement for urinary incontinence or sacrocolpopexy for prolapse repair. Each procedure is tailored to the patient’s needs and health status. Reference: Nygaard, I. E., & Barber, M. D. (2008). Pelvic organ prolapse and stress urinary incontinence in women: A review of surgical treatment. JAMA, 300(10), 1087-1098.
Slide 12-

Postoperative Care and Patient Education Importance of following post-surgical instructions. Tips for recovery: Rest, avoiding heavy lifting, adherence to follow-up. Educating patients on lifestyle adjustments to prevent recurrence. Speaker Notes: Postoperative care is critical for a successful outcome. Patients should avoid strenuous activities and adhere to follow-up visits. Education on lifestyle changes is essential to prevent future issues. Reference: Weber, A. M., & Richter, H. E. (2005). Pelvic organ prolapse and incontinence: Pathophysiology and treatment. Lancet, 369(9566), 1027-1038.
