Introduction to Pregnancy and Chronic Conditions Definition of chronic medical conditions in pregnancy. Importance of managing chronic conditions for maternal and fetal health. Common chronic conditions: diabetes, hypertension, thyroid disorders, and autoimmune diseases. Speaker Notes: Chronic conditions can complicate pregnancy, requiring specialized care to ensure the health and safety of both mother and fetus. Proper management can help minimize risks. Reference: American College of Obstetricians and Gynecologists (ACOG). (2020). Management of chronic medical conditions in pregnancy.

Importance of Preconception Counseling Assessing health status before conception. Optimizing chronic condition management (e.g., medication adjustments). Providing lifestyle recommendations (e.g., diet, exercise). Educating about potential risks and treatment plans. Speaker Notes: Preconception counseling is crucial for women with chronic conditions. It helps identify risks early, so necessary adjustments can be made to ensure a safer pregnancy journey. Reference: Dean, S. V., Imam, A. M., Lassi, Z. S., & Bhutta, Z. A. (2014). Preconception care: Nutritional risks and interventions in women and adolescent girls. Reproductive Health, 11(3).

Managing Diabetes During Pregnancy Types of diabetes in pregnancy: Type 1, Type 2, and gestational diabetes. Monitoring blood glucose levels frequently. Dietary management and lifestyle changes. Insulin adjustments and safe medication use. Risks: macrosomia, preeclampsia, preterm birth. Speaker Notes: Diabetes management during pregnancy is essential to prevent complications for both mother and baby. Frequent glucose monitoring, dietary modifications, and medication adjustments play a key role. Reference: American Diabetes Association. (2019). Standards of medical care in diabetes—pregnancy and diabetes. Diabetes Care, 42(Supplement 1), S165-S172.
Slide 6-

Hypertension and Pregnancy Types: Chronic hypertension, gestational hypertension, preeclampsia. Monitoring blood pressure regularly. Medication safety (e.g., labetalol, nifedipine). Risks: preeclampsia, placental abruption, intrauterine growth restriction (IUGR). Speaker Notes: Hypertension in pregnancy increases the risk of severe complications. Managing blood pressure with safe medications and regular monitoring is essential for preventing adverse outcomes. Reference: Magee, L. A., Pels, A., Helewa, M., Rey, E., & von Dadelszen, P. (2014). Diagnosis, evaluation, and management of the hypertensive disorders of pregnancy. Pregnancy Hypertension, 4(2), 105-145.
Slide 7-

Thyroid Disorders in Pregnancy Types: Hypothyroidism and hyperthyroidism. Monitoring thyroid hormone levels (TSH, T4). Adjusting medications (levothyroxine, antithyroid drugs). Risks of untreated thyroid disorders: preterm birth, miscarriage, developmental delays. Speaker Notes: Thyroid hormone balance is critical for fetal development. Regular monitoring and adjusting medication doses are key to managing thyroid conditions in pregnancy. Reference: Alexander, E. K., Pearce, E. N., Brent, G. A., et al. (2017). Guidelines of the American Thyroid Association for the diagnosis and management of thyroid disease during pregnancy and the postpartum. Thyroid, 27(3), 315-389.
Slide 8-

Autoimmune Diseases and Pregnancy Common autoimmune conditions: Lupus, rheumatoid arthritis, and multiple sclerosis. Medication safety (immunosuppressants, biologics). Risks: preterm birth, IUGR, pregnancy loss. Importance of multidisciplinary care (obstetrician, rheumatologist). Speaker Notes: Autoimmune diseases can complicate pregnancy, often requiring a tailored approach with safe medications and close monitoring. Coordination between specialists is essential for managing these cases. Reference: Lockshin, M. D., Kim, M., & Laskin, C. A. (2011). The management of the pregnant patient with lupus. Lupus, 20(6), 593-597.
Slide 9-

Epilepsy and Pregnancy Managing seizures and medication safety. Risks associated with antiepileptic drugs (birth defects). Monitoring serum drug levels. Preventive care: folic acid supplementation, pre-pregnancy counseling. Speaker Notes: Pregnancy in women with epilepsy requires careful management of seizures and close monitoring of medication levels to balance seizure control with minimizing risks to the fetus. Reference: Tomson, T., Battino, D., Bonizzoni, E., et al. (2015). Antiepileptic drugs and intrauterine death: A prospective observational study from EURAP. Epilepsia, 56(1), 64-71.
Slide 10-

Mental Health and Pregnancy Addressing anxiety and depression in high-risk pregnancies. Psychosocial support and counseling. Safe use of psychiatric medications. Importance of family and community support. Speaker Notes: Chronic conditions can add significant stress to a pregnancy. Mental health support is essential for overall well-being, including counseling and careful selection of medications when necessary. Reference: Yonkers, K. A., Wisner, K. L., Stewart, D. E., et al. (2009). The management of depression during pregnancy: A report from the American Psychiatric Association and the American College of Obstetricians and Gynecologists. Obstetrics & Gynecology, 114(3), 703-713.
Slide 11-

Collaborative Care Approach Importance of a multidisciplinary team (obstetricians, specialists, nutritionists, mental health professionals). Regular check-ups and coordinated care plans. Ensuring continuity of care for maternal and fetal health. Speaker Notes: Collaborative care is essential for managing chronic conditions in pregnancy. A multidisciplinary team ensures that all aspects of maternal and fetal health are addressed, creating a comprehensive care plan. Reference: American College of Obstetricians and Gynecologists (ACOG). (2015). The role of the obstetrician–gynecologist in the collaborative care team for women with chronic conditions.
