Introduction to Perinatal Mental Health Perinatal mental health covers mental well-being from pregnancy through the first year postpartum. Mental health issues in this period affect both mothers and infants, influencing bonding, child development, and overall family health. A holistic approach to maternal health includes mental and emotional support. Speaker Notes: Perinatal mental health is a critical aspect of obstetric care. Providing support for mental well-being during pregnancy and after childbirth positively impacts maternal health, child development, and family dynamics. Reference: Stewart, D. E., & Vigod, S. N. (2016). Perinatal mental health and risk of child maltreatment: A systematic review. Child Abuse & Neglect, 57, 183-193.

Common Perinatal Mental Health Conditions Perinatal Depression: Affects approximately 1 in 7 women. Perinatal Anxiety: Often underdiagnosed, can coexist with depression. Postpartum Depression (PPD): Severe mood changes following childbirth. Postpartum Psychosis: Rare but serious, includes hallucinations and delusions. Speaker Notes: Perinatal depression and anxiety are the most common mental health conditions affecting new mothers. Recognizing and addressing these early can prevent escalation and improve outcomes. Reference: Howard, L. M., & Khalifeh, H. (2020). Perinatal mental health: A review of progress and challenges. World Psychiatry, 19(3), 313-327.

Risk Factors for Perinatal Mental Health Issues Biological Factors: Hormonal changes, previous mental health issues. Psychosocial Factors: Lack of social support, relationship issues, financial stress. Environmental Factors: Traumatic life events, domestic violence, low socioeconomic status. Speaker Notes: Several factors can contribute to perinatal mental health issues. Biological, psychosocial, and environmental factors play a role in increasing vulnerability. Reference: O'Hara, M. W., & McCabe, J. E. (2013). Postpartum depression: Current status and future directions. Annual Review of Clinical Psychology, 9, 379-407.

Signs and Symptoms of Perinatal Mental Health Disorders Persistent sadness, tearfulness, or hopelessness. Extreme fatigue, sleep disturbances, and appetite changes. Irritability or anger, guilt, lack of interest in the baby. Anxiety, panic attacks, and intrusive thoughts. Speaker Notes: Recognizing signs and symptoms early is key. Many women may feel these symptoms but hesitate to seek help due to stigma or lack of awareness. Reference: American College of Obstetricians and Gynecologists (ACOG). (2018). Screening for perinatal depression. Obstetrics & Gynecology, 132(5), e208-e212.

Screening and Diagnosis Routine screening recommended during pregnancy and postpartum. Tools: Edinburgh Postnatal Depression Scale (EPDS), Patient Health Questionnaire (PHQ-9). Importance of non-judgmental environment to encourage honest responses. Speaker Notes: Routine screening can identify perinatal mental health issues early. Using standardized tools like EPDS and PHQ-9 allows providers to assess and monitor patients effectively. Reference: Cox, J. L., Holden, J. M., & Sagovsky, R. (1987). Detection of postnatal depression: Development of the 10-item Edinburgh Postnatal Depression Scale. British Journal of Psychiatry, 150(6), 782-786.

Role of Family and Social Support Encouraging family involvement in patient support. Building a network of support with friends, family, and community resources. Importance of open communication and reducing stigma. Speaker Notes: Family and social support are crucial. Family members can help reduce stress and provide encouragement, allowing the mother to focus on recovery and bonding with the baby. Reference: Dennis, C. L., & Dowswell, T. (2013). Psychosocial and psychological interventions for preventing postpartum depression. Cochrane Database of Systematic Reviews, (2).
Slide 9-

Cultural Sensitivity in Perinatal Mental Health Care Understanding cultural beliefs and norms related to pregnancy and motherhood. Avoiding assumptions; using culturally sensitive language. Recognizing diverse expressions of mental health symptoms. Speaker Notes: Each patient comes from a unique cultural background, which influences how they perceive mental health. Providers should practice cultural sensitivity and understand each patient's individual experience. Reference: World Health Organization (WHO). (2014). Maternal mental health and child health and development in low and middle income countries. Bulletin of the World Health Organization.
Slide 10-

Importance of Early Intervention and Follow-Up Early detection and intervention prevent escalation of symptoms. Regular follow-up visits for monitoring and support. Continuity of care to support ongoing mental health. Speaker Notes: Early intervention can prevent further complications, such as severe depression or anxiety. Regular follow-ups allow healthcare providers to adjust treatment plans as needed. Reference: Murray, L., & Cooper, P. J. (1997). Effects of postnatal depression on infant development. Archives of Disease in Childhood, 77(2), 99-101.
