Objectives Understand the definition and types of Complementary and Alternative Medicine (CAM). Have greater knowledge of the history of CAM and its impact on medical practice. Increase confidence in evaluating health websites. Become more proficient in searching for evidence of the effectiveness of CAM.
Slide 3-

Checking In Let’s take a pre-test to assess your knowledge of CAM.

Defining CAM Complementary: together with aromatherapy to help with pain after surgery Alternative: in place of using garlic to lower blood pressure NCCIH generally uses the term “complementary health approaches” when discussing practices and products of non-mainstream origin.
Slide 7-

Do Definitions Matter? Let’s take a few minutes to talk about why a definition of CAM might be important.
Slide 8-

CAM becomes “legit” 1990 Wilk et al v. AMA 1991 $2 million in funding to establish NIH Office of Alternative Medicine 1994 Dietary Supplement Health and Education Act

CAM becomes “legit” 1995 NIH Office of Dietary Supplements FDA declassifies Acupuncture needles as experimental product 1996 NIH Consensus Conference on Acupuncture 1997 First large trial of CAM therapy, St. John’s Wort for depression

CAM becomes “legit” 1998 National Center for Complementary & Alternative Medicine (NCCAM) established http://nccam.nih.gov/ First full scale article in JAMA on herbal medicine Office of Cancer Complementary and Alternative Medicine (OCCAM) established http://www.cancer.gov/cam/ 2001 CAM on PubMed (NCCAM and NLM) 2009 Results of the first survey from NHIS are published on U.S. spending on CAM. 2015 Congress renames NCCAM the National Center for Complementary and Integrative Health (NCCIH)

Impact of CAM CDC Report (2012) 33.2% of adults used some form of CAM. Children whose parent used CAM were 2x as likely to have used CAM. CAM use more prevalent for: women, adults aged 30-69, higher levels of education, adults who were not poor, adults living in the West, and former smokers. Relationship Between CAM and Western Medicine Let’s take a few minutes to talk about the relationship between CAM and Western medicine. When a therapy is tested and proves to be effective, does it leave the CAM category and become conventional medicine? Sometimes doctors do CAM therapies, and CAM practitioners do conventional therapies. When is it CAM and when is it conventional? Is CAM the appropriate term for medicines that are considered the dominant medical practice in their own country?

CAM Categories Strengths and Weaknesses Let’s talk about the strengths and weaknesses in the CAM categories we reviewed.

Avoiding Bad Science/Pseudoscience The following are some of the indicators of the possible presence of pseudoscience: Use of vague, exaggerated, or untested claims. Over-reliance on confirmation rather than refutation. Lack of openness to testing by other experts. Absence of progress. Use of misleading language. Absence from citation databases.

Examining the Research ClinicalTrials.gov Government and private clinical studies involving humans http://clinicaltrials.gov Observational Studies Clinical Trials/Studies controlled blind/double-blind randomized
Slide 24-

Clinical Trials and CAM Drug companies vs. supplement companies Customization of treatment in CAM CAM does not necessarily want to be studied CAM has only recently become “legit” in the scientific community
Slide 25-

The other side Some say CAM research should not be funded Offit PA. Studying Complementary and Alternative Therapies. JAMA. 2012;307(17):1803–1804. doi:10.1001/jama.2012.518 Or some CAM research should not be funded Salzberg, Steven. Save Taxpayer $$$: Eliminate Alternative Medicine Research. Forbes. 2010; Accessed online 05/22/2018. And others just want more scientific rigor Miller, Henry. NIH needs to raise the bar for funding alternative medicine research. STAT. 2018.
