Ochsner Health System- Orthopedics Division Research Studies Over 100 active studies are currently underway within Orthopedics 20 are led by George Chimento, MD 23 are led by Bradford Waddell, MD Sports department with multiple studies All sections of Orthopedics (Foot/Ankle, Trauma, Peds, etc) have active projects Orthopedics is hoping to include more clinical trials in future research endeavors.

A Retrospective Review of Metal on Metal total hip arthroplasty (PI: George Chimento, MD) RATIONALE AND SIGNIFICANCE Concerns about increased wear and osteolysis in total hip arthroplasty with polyethylene liners have led to a resurgence of interest in metal on metal total hip arthroplasty. The first generation metal on metal implants were abandoned for polyethylene liners secondary to poor materials and implant designs. However, even these implants have shown long term results with survivorship of 93% and newer implants have shown comparative results to metal on polyethylene implants. Metal on polyethylene articulations have shown increased wear rates compared to metal on metal. SPECIFIC AIM To determine whether there is a difference in outcome and complications between metal on metal total hip arthroplasty as compared to metal on polyethylene constructs.

One Stage versus Two Stage For Periprosthetic Hip And Knee Infection (PI: George Chimento, MD) RATIONALE AND SIGNIFICANCE Total joint arthroplasty (TJA) continues to be one of the most successful and cost-effective surgical procedures. Despite its success, TJA is associated with some complications, including infection. Periprosthetic Joint infection (PJI) is a devastating complication following TJA, is associated with marked patient morbidity and mortality, and is an immense economic burden on society. SPECIFIC AIMS To compare the success of one-stage exchange arthroplasty to two-stage exchange arthroplasty for the management of chronic PJI. To compare the financial burden on the healthcare and the Health-Related Quality of Life (HRQOL) of the patients receiving one versus the two-stage exchange arthroplasty.

Joint Replacement Outcomes after Abdominal Organ Transplantation (PI: George Chimento, MD) RATIONALE AND SIGNIFICANCE Joint replacement surgery has been shown to be successful and effective in post solid organ transplantation patients. However, complication rates, revision rates, and overall mortality can be higher in this population compared to patients who have not undergone solid organ transplantation. Changes in immunosuppressive therapy after solid organ transplantation have decreased the rate of avascular necrosis (AVN) of both the hip and the knee. However, transplant patients are still more prone to this joint destroying disease2. SPECIFIC AIMS To compare outcomes between solid organ transplant recipients who have undergone joint replacement surgery and the general population undergoing joint replacement surgery. To determine a specific length of time between the two surgeries that is most beneficial to outcomes. To determine which joint replacement procedure is more prone to complications with regards to solid organ transplant patients

Prospective Evaluation of a Non-invasive Hemoglobin Measurement System in Total Joint Arthroplasty (PI: Bradford Waddell, MD) RATIONALE AND SIGNIFICANCE In patients undergoing major orthopedic surgical procedures, monitoring post-operative hemoglobin and hematocrit is essential. Traditionally, this is done via laboratory analysis of blood drawn from a patient’s IV. The Masimo SpHb system was designed as an alternative to a traditional blood draw. Prior studies have shown this system to be accurate and effective. SPECIFIC AIMS To determine the accuracy of the non-invasive hemoglobin measurement system (Masimo SpHb) compared with a traditional laboratory-measured blood draw. To determine the potential financial savings associated with the use of this device.
