About This Presentation

Description : Oncofertility: Merging Cancer Care and Fertility Preservation
Oncofertility is an innovative field that bridges oncology and reproductive medicine, offering cancer patients the hope of preserving fertility during and after treatment. With improved cancer survival rates, ensuring quality of life—including the ability to have biological children—has become an essential aspect of cancer care.
Cancer treatments such as chemotherapy, radiation, and surgery can significantly impact fertility in men, women, and even pediatric patients. Oncofertility provides various preservation techniques, including egg and sperm cryopreservation, embryo freezing, ovarian and testicular tissue freezing, and fertility-sparing surgeries. These options empower patients to safeguard their reproductive potential, even in the face of a cancer diagnosis.
The presentation highlights challenges like ethical considerations, costs, and the emotional toll of fertility loss, as well as emerging technologies such as artificial ovaries and testicular organoids that promise to revolutionize fertility preservation. With a focus on patient-centered care, oncofertility ensures that patients’ future family-building dreams are part of the conversation from the start.
By integrating reproductive medicine into cancer care, oncofertility creates a bridge to a brighter future for cancer survivors, addressing both survival and long-term quality of life.
Take the step to learn about how this transformative field is changing lives.

Oncofertility: Bridging Oncology and Reproductive Medicine Preserving Fertility Amidst Cancer Care Dr. Vineeta Bose Oncologist & Hematologist

What is Oncofertility? Oncofertility is a field that integrates oncology and reproductive medicine. It focuses on preserving fertility in cancer patients before, during, and after treatment. Growing need due to improved cancer survival rates and younger age at diagnosis. Speaker Notes: Oncofertility emerged as a response to cancer treatments like chemotherapy and radiation, which often impair fertility. It reflects the increasing importance of quality of life post-cancer. Reference: Woodruff, T. K. (2010). The Oncofertility Consortium—addressing fertility in young people with cancer. Nature Reviews Clinical Oncology, 7(8), 466-475.

Cancer Treatments and Their Impact on Fertility Chemotherapy: Damages ovarian reserve or sperm production. Radiation Therapy: May harm reproductive organs and hormone production. Surgery: Removal of reproductive organs may eliminate fertility potential. Targeted Therapy: Some drugs may have less impact but still carry risks. Speaker Notes: The severity of fertility impairment depends on the type of cancer, treatment, and patient’s age. Reference: Meirow, D., & Nugent, D. (2001). The effects of radiotherapy and chemotherapy on female reproduction. Human Reproduction Update, 7(6), 535-543.

Fertility Preservation Options for Women Egg and Embryo Freezing (Cryopreservation): Gold standard for fertility preservation. Requires hormonal stimulation and retrieval of eggs. Ovarian Tissue Cryopreservation: Experimental but promising, especially for prepubescent patients. Ovarian Suppression: Use of GnRH agonists to protect ovaries during chemotherapy. Fertility-Sparing Surgeries: Preserving the uterus or part of the ovary. Speaker Notes: While embryo and egg freezing are well-established, ovarian tissue cryopreservation is particularly relevant for young girls or patients who cannot delay treatment. Reference: Donnez, J., & Dolmans, M. M. (2017). Fertility preservation in women. New England Journal of Medicine, 377(17), 1657-1665.

Fertility Preservation Options for Men Sperm Cryopreservation: Simple, effective, and widely available. Testicular Tissue Freezing: Experimental technique, primarily for prepubescent boys. Testicular Shielding: Reduces exposure during radiation therapy. Fertility-Sparing Surgeries: Avoiding damage to reproductive organs during cancer-related surgeries. Speaker Notes: Sperm cryopreservation is the standard method for preserving fertility in men, but advances like testicular tissue freezing provide hope for pediatric patients. Reference: Wyns, C. (2010). Fertility preservation in the male pediatric population: Current achievements and research challenges. Pediatric Endocrinology Reviews, 7(4), 386-394.

Oncofertility in Pediatric Patients Challenges: Prepubescent children cannot produce eggs or sperm. Ethical concerns about consent. Solutions: Ovarian and testicular tissue freezing. Advances in in-vitro maturation (IVM) and tissue re-implantation. Parental Involvement: Educating families about future fertility options. Speaker Notes: For pediatric patients, experimental approaches like ovarian/testicular tissue freezing offer hope, but challenges in ethical and biological aspects remain. Reference: Wallace, W. H., & Kelsey, T. W. (2010). Fertility preservation in children treated for cancer. Nature Reviews Clinical Oncology, 7(8), 409-419.

Ethical Considerations in Oncofertility Informed consent for minors and young adults. Cost and accessibility of fertility preservation techniques. Balancing urgency of cancer treatment with fertility preservation options. Long-term implications of preserved tissues. Speaker Notes: Ethical considerations must address cost, accessibility, and consent to ensure equitable oncofertility care for all patients. Reference: Oktay, K., Harvey, B. E., & Partridge, A. H. (2018). Fertility preservation in patients with cancer: ASCO clinical practice guideline update. Journal of Clinical Oncology, 36(19), 1994-2001.

Psychological Impact of Fertility Loss Fertility loss can lead to anxiety, depression, and grief. Importance of counseling and emotional support. Role of support groups in helping patients cope. Speaker Notes: Losing the ability to have biological children can be a deeply distressing consequence of cancer treatment. Counseling is essential. Reference: Letourneau, J. M., Ebbel, E. E., Katz, P. P., et al. (2012). Acute ovarian failure underestimates age-specific reproductive impairment for young women undergoing chemotherapy for cancer. Cancer, 118(7), 1933-1939.

Advances in Oncofertility Research Improved techniques for ovarian and testicular tissue cryopreservation. Artificial ovaries and testicular organoids. Integration of artificial intelligence (AI) in fertility prediction and preservation. Increasing access and affordability. Speaker Notes: Advances like artificial ovaries and testicular tissue engineering hold promise for transforming fertility preservation. Reference: Martinez, F. (2017). Update on fertility preservation from the Barcelona International Society for Fertility Preservation–ESHRE–ASRM 2015 expert meeting: Indications, results, and future perspectives.
