What is Polycystic Ovary Syndrome (PCOS)? PCOS is a common hormonal disorder affecting women of reproductive age. Characterized by irregular menstrual cycles, excessive androgen levels, and polycystic ovaries. Affects approximately 5-10% of women worldwide. Speaker Notes: PCOS is a complex condition that can affect a woman’s menstrual cycle, fertility, appearance, and overall health. It is one of the most common endocrine disorders among women of reproductive age. Reference: Azziz, R., et al. (2004). The prevalence and features of the polycystic ovary syndrome in an unselected population. The Journal of Clinical Endocrinology & Metabolism, 89(6), 2745-2749.

Common Symptoms of PCOS Irregular periods or no periods at all. Excess androgen (male hormone) causing hirsutism, acne, and hair loss. Weight gain or difficulty losing weight. Ovarian cysts. Skin issues (dark patches, skin tags). Speaker Notes: PCOS symptoms can vary greatly from one individual to another. While some women experience irregular periods and weight gain, others may struggle with severe acne and hair loss due to increased androgen levels. Reference: Teede, H. J., et al. (2010). Assessment and management of polycystic ovary syndrome: summary of an evidence-based guideline. Medical Journal of Australia, 195(6), 65-112.

Causes and Risk Factors Genetic predisposition: Family history of PCOS or diabetes. Insulin resistance: Commonly associated with PCOS. Lifestyle factors: Sedentary lifestyle, poor diet can exacerbate symptoms. Hormonal imbalance: Increased levels of androgens and luteinizing hormone (LH). Speaker Notes: The exact cause of PCOS is unknown, but genetic and lifestyle factors contribute significantly. Insulin resistance and hormonal imbalances are often associated with PCOS, leading to a cascade of symptoms. Reference: Dunaif, A. (1997). Insulin resistance and the polycystic ovary syndrome: mechanism and implications for pathogenesis. Endocrine Reviews, 18(6), 774-800.

Diagnosis of PCOS Based on Rotterdam Criteria (2003): Requires two out of three of the following: Irregular or absent menstrual cycles. Clinical or biochemical signs of hyperandrogenism. Polycystic ovaries visible on ultrasound. Exclusion of other conditions (e.g., thyroid dysfunction, adrenal hyperplasia). Speaker Notes: Diagnosis is based on the Rotterdam criteria, which requires at least two of three features: irregular periods, excess androgens, and polycystic ovaries. It’s also important to rule out other conditions with similar symptoms. Reference: Rotterdam ESHRE/ASRM-Sponsored PCOS Consensus Workshop Group (2004). Revised 2003 consensus on diagnostic criteria and long-term health risks related to polycystic ovary syndrome (PCOS). Human Reproduction, 19(1), 41-47.

Long-Term Health Risks of PCOS Increased risk of Type 2 diabetes. Higher chances of metabolic syndrome and cardiovascular disease. Greater risk of endometrial cancer due to irregular cycles. Psychological issues like depression and anxiety. Speaker Notes: Beyond reproductive health, PCOS can lead to several long-term health risks, including diabetes, cardiovascular disease, and endometrial cancer. It’s essential to monitor and manage these risks proactively. Reference: Ehrmann, D. A. (2005). Polycystic ovary syndrome. New England Journal of Medicine, 352(12), 1223-1236.

Treatment Options for PCOS Lifestyle modifications: Weight loss, exercise, and diet improvements. Medications: Hormonal treatments: Birth control pills to regulate cycles. Metformin: For insulin resistance. Anti-androgens: To reduce excess hair growth. Fertility treatments: Clomiphene, letrozole, and assisted reproductive technology (ART) for those wishing to conceive. Speaker Notes: Treatment focuses on managing symptoms and varies based on the patient's goals, such as improving fertility or controlling symptoms like hirsutism and irregular periods. Lifestyle changes are foundational, but medications like metformin and birth control pills can be highly effective. Reference: Legro, R. S., et al. (2013). Diagnosis and treatment of polycystic ovary syndrome: an Endocrine Society clinical practice guideline. The Journal of Clinical Endocrinology & Metabolism, 98(12), 4565-4592.
Slide 9-

Lifestyle Modifications for PCOS Management Dietary changes: Focus on low glycemic index foods. Regular physical activity: Helps with insulin sensitivity and weight management. Stress management: Practices like mindfulness and yoga. Sleep hygiene: Regulating sleep patterns can improve hormonal balance. Speaker Notes: Lifestyle modifications are crucial in managing PCOS. A balanced diet, regular exercise, and stress reduction are essential steps in controlling insulin resistance and improving symptoms. Reference: Moran, L. J., et al. (2011). Dietary composition in the treatment of polycystic ovary syndrome: a systematic review to inform evidence-based guidelines. Journal of the Academy of Nutrition and Dietetics, 111(5), 629-640.
Slide 10-

Psychological and Emotional Impact of PCOS Increased prevalence of depression and anxiety. Body image issues due to symptoms like weight gain and hirsutism. Importance of support groups and mental health counseling. Speaker Notes: Many women with PCOS experience emotional challenges due to symptoms that affect their appearance and self-esteem. Mental health support is essential in PCOS management. Reference: Bazarganipour, F., et al. (2013). Health-related quality of life in patients with polycystic ovary syndrome (PCOS): a model-based study. Journal of Ovarian Research, 6(1), 44.
