Management of Metastatic Disease Principles, Treatment Strategies, and Supportive Care Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

What is Metastatic Disease? Definition: Metastatic disease occurs when cancer spreads from the primary site to distant organs or tissues. Common sites of metastases: Bones, liver, lungs, brain. Key Challenge: Management focuses on symptom control, extending survival, and maintaining quality of life. Speaker Notes: Metastatic disease represents an advanced stage of cancer. While it is often incurable, systemic therapies and supportive care have significantly improved outcomes. Reference: Siegel, R. L., Miller, K. D., & Jemal, A. (2020). Cancer statistics, 2020. CA: A Cancer Journal for Clinicians.

Goals of Management Control Disease Progression: Slow tumor growth using systemic therapies. Symptom Management: Relieve pain, fatigue, and organ dysfunction. Improve Survival: Extend life expectancy while maintaining quality of life. Support Psychosocial Well-being: Address emotional and social aspects. Speaker Notes: The primary goal is not always curative but to balance disease control with quality of life. Reference: Hellman, S., & Weichselbaum, R. R. (1995). Oligometastases. Journal of Clinical Oncology.

Multidisciplinary Approach Team Members: Oncologists, radiologists, surgeons, pathologists, palliative care specialists, and mental health counselors. Coordination ensures: Comprehensive treatment planning. Holistic patient support. Tailored therapies for better outcomes. Speaker Notes: Collaboration across specialties is key to providing the best care for patients with metastatic disease. Reference: Abdel-Rahman, O., & Fouad, M. (2021). Multidisciplinary approaches in metastatic cancer care. Cancer Treatment Reviews.

Systemic Therapy for Metastatic Disease Chemotherapy: Used in many metastatic cancers (e.g., lung, breast, colorectal). Goal: Cytotoxic effect to shrink or slow tumor growth. Targeted Therapy: Focuses on molecular targets (e.g., HER2 in breast cancer, EGFR in lung cancer). Examples: Trastuzumab, Erlotinib. Immunotherapy: Boosts immune response against cancer cells. Examples: Checkpoint inhibitors (nivolumab, pembrolizumab). Hormonal Therapy: For hormone-sensitive cancers (e.g., breast, prostate cancer). Examples: Tamoxifen, Androgen Deprivation Therapy. Speaker Notes: Systemic therapies are the backbone of metastatic disease management, with personalized approaches based on tumor biology. Reference: Hanna, N., Johnson, D., & Temin, S. (2020). Systemic Therapy for Advanced Non–Small-Cell Lung Cancer. Journal of Clinical Oncology.

Site-Specific Metastatic Management Bone Metastases: Pain relief: Radiation therapy. Bisphosphonates (e.g., zoledronic acid) and denosumab to prevent fractures. Liver Metastases: Systemic therapy or hepatic artery infusion chemotherapy. Selective cases: Liver resection or ablation. Brain Metastases: Whole brain radiation therapy (WBRT), stereotactic radiosurgery (SRS). Corticosteroids for edema. Lung Metastases: Surgical resection in oligometastatic cases. Symptom control with radiation therapy. Speaker Notes: Management varies depending on the metastatic site, with a focus on local control and symptom management. Reference: Coleman, R. E. (2006). Clinical features of metastatic bone disease and risk of skeletal morbidity. Clinical Cancer Research.

Role of Palliative Care Symptom management: Pain, nausea, fatigue, breathlessness. Emotional and psychosocial support. Enhancing quality of life for patients and families. Early integration with oncologic treatment improves outcomes. Speaker Notes: Palliative care focuses on improving the patient’s overall experience and should be initiated early in metastatic disease management. Reference: Temel, J. S., Greer, J. A., & Muzikansky, A. (2010). Early palliative care for patients with metastatic non–small-cell lung cancer. New England Journal of Medicine.

Advances in Management Liquid biopsies for monitoring tumor genetics. Personalized medicine based on genetic profiling. Role of artificial intelligence in treatment planning. New immunotherapies: CAR-T cell therapy and bispecific antibodies. Speaker Notes: Advances in molecular biology and technology are revolutionizing metastatic cancer management with more personalized and effective therapies. Reference: Zhang, J., Wang, H., & Wang, Z. (2019). Advances in liquid biopsy for metastatic cancer. Cancer Biology & Medicine.

Challenges in Metastatic Disease Management Development of drug resistance. Balancing quality of life with aggressive treatments. Financial toxicity for patients and families. Addressing patient and caregiver emotional needs. Speaker Notes: Despite advancements, metastatic disease management presents significant challenges requiring innovative approaches and compassionate care. Reference: Longo, D. L., & DeVita, V. T. (2015). The role of systemic therapy in metastatic cancer. Cancer Journal.

Key Takeaways Metastatic disease requires a multidisciplinary and personalized approach. Goals include prolonging survival, relieving symptoms, and enhancing quality of life. Emerging therapies and advances offer hope but must balance efficacy with patient well-being. Speaker Notes: Effective management of metastatic disease focuses not only on prolonging life but also on ensuring that patients can live well during treatment.
