Emerging Therapies in Medical Oncology Innovative Treatments Shaping the Future of Cancer Care Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

Advancements in Medical Oncology Traditional therapies (chemotherapy, radiation) are being complemented by cutting-edge treatments. Focus on targeted, personalized, and immune-based therapies. Goal: Improve patient outcomes, reduce toxicity, and address resistant cancers. Speaker Notes: This presentation explores new approaches that are revolutionizing cancer care, including immunotherapies, targeted treatments, and emerging technologies like nanomedicine. Reference: Siegel, R. L., Miller, K. D., & Jemal, A. (2020). Cancer statistics, 2020. CA: A Cancer Journal for Clinicians.

Immunotherapy: Harnessing the Immune System Checkpoint Inhibitors: Block immune checkpoints (e.g., PD-1/PD-L1 inhibitors like pembrolizumab, nivolumab). CAR-T Cell Therapy: Genetically engineered T-cells target specific cancer cells (e.g., for lymphoma, leukemia). Tumor Vaccines: Therapeutic vaccines like sipuleucel-T for prostate cancer. Speaker Notes: Immunotherapy works by empowering the immune system to recognize and destroy cancer cells, offering long-term remission for many patients. Reference: Ribas, A., & Wolchok, J. D. (2018). Cancer immunotherapy using checkpoint blockade. Science, 359(6382), 1350-1355.

Precision Medicine with Targeted Therapies Drugs targeting cancer-specific molecules or pathways: EGFR Inhibitors (e.g., erlotinib for lung cancer). HER2 Inhibitors (e.g., trastuzumab for breast cancer). BRAF/MEK Inhibitors (e.g., dabrafenib for melanoma). Focus on minimizing effects on normal cells. Role of genetic and molecular profiling in treatment selection. Speaker Notes: Targeted therapies identify and attack specific cancer cells based on genetic or molecular features, making treatments more effective and less toxic. Reference: Druker, B. J., et al. (2001). Efficacy of a specific inhibitor of the BCR-ABL tyrosine kinase in chronic myeloid leukemia. New England Journal of Medicine, 344(14), 1031-1037.

Antibody-Drug Conjugates (ADCs) Mechanism: Antibodies deliver cytotoxic drugs directly to cancer cells. Examples: Trastuzumab emtansine (T-DM1) for HER2-positive breast cancer. Brentuximab vedotin for Hodgkin lymphoma. Advantages: Targeted delivery reduces systemic toxicity. Speaker Notes: ADCs combine the specificity of antibodies with the potency of cytotoxic agents, enhancing effectiveness while reducing harm to healthy tissues. Reference: Beck, A., et al. (2017). Strategies and challenges for the next generation of antibody-drug conjugates. Nature Reviews Drug Discovery, 16(5), 315-337.

Bispecific Antibodies Mechanism: Bind two different antigens simultaneously. Example: CD19 on cancer cells and CD3 on T-cells to activate immune attack. Example drug: Blinatumomab for acute lymphoblastic leukemia (ALL). Novel approach for hard-to-treat cancers. Speaker Notes: Bispecific antibodies act like a "bridge," connecting immune cells to cancer cells, improving the immune system’s ability to target and destroy tumors. Reference: rinkmann, U., & Kontermann, R. E. (2017). The making of bispecific antibodies. mAbs, 9(2), 182-212.

Epigenetic Therapy Mechanism: Targeting reversible epigenetic changes like DNA methylation and histone modifications. Drugs: Azacitidine and Decitabine for myelodysplastic syndromes. HDAC inhibitors (e.g., vorinostat) for T-cell lymphomas. Future direction: Personalized epigenetic therapy based on tumor profiling. Speaker Notes: Epigenetic therapy is a cutting-edge approach targeting changes in gene expression without altering the DNA sequence, restoring normal function in cancer cells. Reference: Jones, P. A., & Baylin, S. B. (2007). The epigenomics of cancer. Cell, 128(4), 683-692.
Slide 8-

Nanomedicine in Oncology Mechanism: Using nanoparticles to deliver drugs directly to tumors. Benefits: Enhanced precision, reduced side effects. Examples: Doxil (liposomal doxorubicin) for ovarian cancer. Nanoparticles for combination drug delivery and imaging. Speaker Notes: Nanomedicine enhances the efficacy of cancer therapies by improving drug delivery systems, reducing systemic toxicity, and enabling imaging for precision treatment. Reference: Shi, J., et al. (2017). Cancer nanomedicine: Progress, challenges, and opportunities. Nature Reviews Cancer, 17(1), 20-37.
Slide 9-

Synergy in Combination Therapy Combining therapies to enhance efficacy and overcome resistance: Immunotherapy + Targeted Therapy. Chemotherapy + PARP Inhibitors. Examples: Pembrolizumab + Axitinib for renal cell carcinoma. Olaparib + bevacizumab for ovarian cancer. Speaker Notes: Combining therapies leverages the strengths of each modality, creating synergy to attack cancer cells from multiple angles while minimizing resistance. Reference: Al-Lazikani, B., Banerji, U., & Workman, P. (2012). Combinatorial drug therapy for cancer in the post-genomic era. Nature Biotechnology, 30(7), 679-692.
Slide 10-

Challenges in Emerging Therapies High cost and accessibility of therapies. Managing side effects (e.g., immune-related toxicities in immunotherapy). Overcoming drug resistance. Need for large-scale clinical trials for new approaches. Speaker Notes: While emerging therapies offer hope, challenges such as cost, side effects, and resistance need to be addressed through further research and innovation. Reference: Wong, C. H., Siah, K. W., & Lo, A. W. (2019). Estimating the cost of drug development: A review. JAMA, 323(9), 844-854.

Conclusion Emerging therapies are transforming cancer treatment by improving outcomes and quality of life. Personalized medicine and innovative technologies are at the forefront. Collaboration between researchers, clinicians, and policymakers is essential. Speaker Notes: The future of oncology lies in precision medicine, innovative therapies, and ensuring equitable access to these advancements.
