About This Presentation

Description : Infertility and Assisted Reproductive Technologies (ART) in Obstetrics
This presentation on "Infertility and Assisted Reproductive Technologies (ART)" provides a comprehensive overview of infertility and the modern advancements in reproductive technology. Infertility affects millions of couples worldwide, and Assisted Reproductive Technologies, or ART, offer hope and options for those struggling to conceive.
The presentation covers the causes of infertility in both men and women, including ovulatory disorders, endometriosis, and low sperm count, while also addressing the complexities of unexplained infertility. Diagnostic methods are discussed in detail, from hormonal testing to semen analysis, to help healthcare providers and patients understand the underlying issues.
Various ART options such as In Vitro Fertilization (IVF), Intracytoplasmic Sperm Injection (ICSI), and Intrauterine Insemination (IUI) are highlighted, alongside their processes, success rates, and potential risks. Additionally, the presentation delves into other options like egg donation, surrogacy, and embryo freezing.
Ethical and social considerations surrounding ART are also addressed, providing a well-rounded view of the emotional, financial, and legal implications of reproductive technology. This presentation is ideal for healthcare professionals and patients seeking insights into modern fertility treatments.

Introduction to Infertility and ART Infertility is the inability to conceive after 12 months of regular, unprotected intercourse. Affects 10-15% of couples globally. Assisted Reproductive Technologies (ART) are treatments aimed at helping couples achieve pregnancy. Speaker Notes: Infertility affects millions of people worldwide and can be emotionally challenging. ART has transformed the ability of couples to conceive, offering hope and options for those facing infertility. Reference: Practice Committee of the American Society for Reproductive Medicine. (2013). Definitions of infertility and recurrent pregnancy loss: a committee opinion. Fertility and Sterility, 99(1), 63.

Causes of Infertility Female Factors (40-50%): Ovulatory disorders, endometriosis, tubal blockage. Male Factors (30-40%): Low sperm count, motility issues, genetic factors. Unexplained Infertility (10-20%): No identifiable cause despite thorough testing. Speaker Notes: Infertility can be caused by a range of factors affecting both men and women, making it essential to perform a comprehensive evaluation on both partners. Reference: Boivin, J., Bunting, L., Collins, J. A., & Nygren, K. G. (2007). International estimates of infertility prevalence and treatment-seeking: potential need and demand for infertility medical care. Human Reproduction, 22(6), 1506-1512.

Diagnostic Methods in Infertility Female Tests: Ovulation tracking, hormonal testing, Hysterosalpingography (HSG), laparoscopy. Male Tests: Semen analysis, hormone testing, testicular biopsy. Additional Tests: Genetic testing, karyotyping for recurrent pregnancy loss. Speaker Notes: A thorough diagnostic workup is essential to identify the underlying cause of infertility. Tests are tailored based on patient history and findings from initial examinations. Reference: World Health Organization. (2010). WHO manual for the standardized investigation and diagnosis of the infertile couple. Cambridge University Press.

Overview of Assisted Reproductive Technologies (ART) ART includes medical treatments that aid fertilization outside the natural process. Common ART procedures: In Vitro Fertilization (IVF), Intracytoplasmic Sperm Injection (ICSI), Intrauterine Insemination (IUI), and others. Speaker Notes: ART covers a variety of treatments, each designed to help overcome different infertility issues. IVF and ICSI are the most widely known and utilized ART procedures. Reference: American Society for Reproductive Medicine. (2020). Assisted reproductive technologies: A guide for patients. ASRM.
Slide 7-

In Vitro Fertilization (IVF) Process: Ovarian stimulation, egg retrieval, fertilization in lab, embryo transfer. Success Rates: Vary by age, generally 40-50% for women under 35, decreasing with age. Risks: Multiple births, ovarian hyperstimulation syndrome (OHSS), ectopic pregnancy. Speaker Notes: IVF is one of the most common and effective forms of ART. However, its success depends on many factors, including maternal age and egg quality. Reference: Centers for Disease Control and Prevention (CDC). (2021). Assisted Reproductive Technology (ART) Success Rates.
Slide 8-

Intracytoplasmic Sperm Injection (ICSI) Procedure: Injection of a single sperm directly into an egg. Indications: Male infertility (low sperm count, motility issues). Often combined with IVF for better outcomes in male factor infertility. Speaker Notes: ICSI has revolutionized the treatment of male infertility and is often used when IVF alone may not be sufficient. Reference: Palermo, G., Joris, H., Devroey, P., & Van Steirteghem, A. C. (1992). Pregnancies after intracytoplasmic injection of single spermatozoon into an oocyte. The Lancet, 340(8810), 17-18.
Slide 9-

Intrauterine Insemination (IUI) Process: Sperm is washed, concentrated, and placed directly into the uterus. Indications: Mild male infertility, unexplained infertility, or when intercourse is difficult. Success Rate: Varies from 10-20% per cycle. Speaker Notes: IUI is a less invasive option than IVF, with a lower cost but also lower success rates, making it suitable for mild infertility cases. Reference: Allen, V. M., Wilson, R. D., & Cheung, A. (2006). Pregnancy outcomes after assisted reproductive technology. Journal of Obstetrics and Gynaecology Canada, 28(3), 220-233.
Slide 10-

Other ART Options Egg/Sperm Donation: For patients with poor gamete quality or genetic concerns. Surrogacy: When a woman cannot carry a pregnancy herself. Cryopreservation: Freezing eggs, sperm, or embryos for future use. Speaker Notes: These options provide alternatives for couples facing unique reproductive challenges. Surrogacy and egg/sperm donation can open doors to parenthood for many. Reference: Ethics Committee of the American Society for Reproductive Medicine. (2013). Third-party reproduction: sperm, egg, and embryo donation and surrogacy. Fertility and Sterility, 100(3), 630-637.
Slide 11-

Ethical and Social Considerations Ethical Issues: Genetic selection, surrogacy ethics, embryo disposition. Psychosocial Aspects: Stress, emotional strain, societal expectations. Legal Aspects: Varying laws on ART, surrogacy, and donor anonymity. Speaker Notes: ART comes with ethical and social complexities, including decisions about embryo use and considerations for mental well-being in couples undergoing treatment. Reference: Shenfield, F., Pennings, G., Cohen, J., Devroey, P., de Wert, G., & Tarlatzis, B. (2005). Taskforce 5: ethics of medically assisted fertility treatment for HIV-positive men and women. Human Reproduction, 20(8), 1974-1980.
Slide 12-

Success Rates and Limitations of ART Success varies by age, health, type of ART. ART can be physically, emotionally, and financially demanding. Importance of setting realistic expectations. Speaker Notes: While ART has helped many couples conceive, it is not a guaranteed solution. Success depends on multiple factors, and it’s essential to set realistic expectations for patients. Reference: European Society of Human Reproduction and Embryology. (2019). ART success rates by age. ESHRE.
