1. Paronychia: Acute. Chronic. 2. Felon. 3. Herpetic Whitlow. 4. Flexor Sheath Infections. 5. Palmar Infections. 6. Dorsal Infections. 7. ‘Fight Bites’
Slide 3-

Overview Most commonly staph aureus Many infections due to multiple organisms 30-40% grow anaerobic species. Other organisms include streptococci, enterobacteria, pseudomonas, enterococci, bacteroides.

Paronychia
Slide 5-

Acute Paronychia Usually presents two to five days post-trauma. Usually Staphylococcus aureus and Streptococcus pyogenes Treatment: Warm water soaks if an abscess has not formed.

Chronic Paronychia Chronic inflammatory condition (>6/52) Causes thickening of the cuticle and grooving of the nail Most Commonly caused by a candidal infection Often described in middle-aged women
Slide 8-

Conservative: topical antifungal/steroid. Surgical treatment: marsupialisation of perionychium Nail removal and topical antifungal
Slide 9-

Felon Abscess of the distal pulp or phalanx pad of the fingertip.
Slide 10-

Usually penetrating trauma Most commonly affected digits are the thumb and index finger May arise when an untreated paronychia spreads into the pad of the fingertip.

Herpetic Whitlow Results from autoinoculation of type 1 or type 2 herpes simplex virus into broken skin. The infection may occur as a complication of primary oral or genital herpes lesions. Health care workers exposed to oral secretions
Slide 13-

Fever, lymphadenitis, lymphadenopathy Small, clear vesicles initially are present.
Slide 14-

Flexor Sheath Infections
Slide 15-

Kanavel’s four cardinal signs: (1) uniform, symmetric digit swelling; (2) at rest, digit is held in partial flexion; (3) excessive tenderness along the entire course of the flexor tendon sheath; (4) pain along the tendon sheath with passive digit extension. Pain with passive extension is the most clinically reproducible
Slide 16-

Usually secondary trauma or puncture wound. The trauma often is at a flexor crease; this is where the tendon sheath is most superficial.
Slide 17-

Staphylococcus and Streptococcus Early: splinting, elevation, ivAx 12-24 hours no improvement → surgical intervention Early surgery in immunocompromised or diabetics.
