About This Presentation

Description : This presentation on "Gynecological Emergencies: Acute Pelvic Pain" offers a comprehensive overview of the causes, diagnostic approaches, and management strategies for acute pelvic pain in gynecological emergencies. Acute pelvic pain can signal serious underlying conditions that require immediate attention. This presentation focuses on critical gynecological emergencies, including ectopic pregnancy, ovarian torsion, pelvic inflammatory disease (PID), ruptured ovarian cysts, and endometriosis-related pain.
Each condition is explored in detail, covering symptoms, risk factors, diagnostic tools (such as ultrasound, serum ß-hCG levels, and laparoscopy), and treatment options ranging from medication to surgical intervention. The presentation also provides visual diagrams and flowcharts for a clear understanding of complex conditions and their management pathways.
Designed for medical students, healthcare professionals, and individuals interested in gynecological health, this presentation emphasizes the importance of early diagnosis and prompt intervention to prevent complications. It also includes a diagnostic approach guide and pathways for managing stable and unstable patients. Join us in exploring the essentials of gynecological emergency care, with practical insights and up-to-date references to support effective patient management and improve outcomes in critical cases.

Introduction to Acute Pelvic Pain Definition: Acute pelvic pain is sudden-onset pain in the lower abdomen or pelvis, usually lasting less than three months. Importance: Pelvic pain can be a symptom of serious gynecological emergencies requiring prompt diagnosis and intervention. Objective: This presentation covers common causes, diagnostic methods, and management of acute pelvic pain in gynecology. Reference: Johnson N, et al. "Emergency management of acute pelvic pain." Obstetrics & Gynecology 2021. ACOG Practice Bulletin. "Evaluation of acute pelvic pain." American College of Obstetricians and Gynecologists, 2020.

Common Causes of Acute Pelvic Pain in Gynecology Ectopic Pregnancy Ovarian Torsion Pelvic Inflammatory Disease (PID) Ruptured Ovarian Cyst Endometriosis Reference: Haskins J, et al. "Acute pelvic pain: Differential diagnosis and evaluation." American Family Physician, 2020.
Slide 5-

Ectopic Pregnancy Definition: Implantation of a fertilized egg outside the uterine cavity, commonly in the fallopian tube. Symptoms: Severe unilateral pelvic pain, vaginal bleeding, shoulder pain (referred pain). Diagnostic Tools: Ultrasound, serum β-hCG levels. Management: Methotrexate for stable cases, surgical intervention (laparotomy or laparoscopy) for ruptured ectopic pregnancies. Reference: Barnhart K, et al. "Ectopic pregnancy." The New England Journal of Medicine, 2019.
Slide 6-

Ovarian Torsion Definition: Twisting of the ovary on its supporting ligaments, which can restrict blood flow. Symptoms: Sudden, severe, unilateral pelvic pain, nausea, vomiting. Diagnostic Tools: Doppler ultrasound, which may show decreased blood flow to the affected ovary. Management: Surgical detorsion or oophorectomy if necrosis is present. Reference: Ghosh T, et al. "Ovarian torsion: Clinical and imaging features." Ultrasound Quarterly, 2021.
Slide 7-

Pelvic Inflammatory Disease (PID) Definition: Infection of the female reproductive organs, often due to sexually transmitted infections (STIs) like Chlamydia or Gonorrhea. Symptoms: Bilateral lower abdominal pain, fever, abnormal vaginal discharge, pain during intercourse. Diagnostic Tools: Clinical examination, cervical swabs, ultrasound for tubo-ovarian abscess. Management: Antibiotics for mild to moderate cases, surgical intervention for abscesses or severe cases. Reference: CDC. "Pelvic Inflammatory Disease (PID): Diagnosis and treatment guidelines." Centers for Disease Control and Prevention, 2020.
Slide 8-

Ruptured Ovarian Cyst Definition: A cyst on the ovary that bursts, causing fluid release and irritation. Symptoms: Sudden sharp or stabbing pain, often unilateral, nausea, possible lightheadedness or dizziness. Diagnostic Tools: Ultrasound to detect free fluid in the pelvis. Management: Pain management for stable cases, surgical intervention for hemorrhage or peritonitis. Reference: Razavi T, et al. "Management of ovarian cysts." Journal of Gynecology Surgery, 2021.
Slide 9-

Endometriosis-Related Acute Pelvic Pain Definition: Growth of endometrial tissue outside the uterus, leading to inflammation and pain. Symptoms: Chronic pelvic pain that can intensify suddenly, dysmenorrhea, dyspareunia (pain during intercourse). Diagnostic Tools: Ultrasound, MRI, laparoscopy for direct visualization. Management: NSAIDs, hormonal therapy, surgical resection of endometriotic lesions in severe cases. Reference: Giudice LC. "Endometriosis." The New England Journal of Medicine, 2019.
Slide 10-

Diagnostic Approach to Acute Pelvic Pain Initial Assessment: History, physical examination, and pain characterization. Laboratory Tests: Serum β-hCG for pregnancy, CBC, inflammatory markers. Imaging Studies: Ultrasound as the first-line imaging tool, CT or MRI if needed. Reference: ACOG Practice Bulletin. "Approach to acute pelvic pain." American College of Obstetricians and Gynecologists, 2020.
Slide 11-

Management and Treatment Pathways Stable Patients: Observation, analgesics, antibiotics (for PID). Unstable Patients: Emergency surgical intervention, blood transfusion if needed. Patient Education: Importance of follow-up, preventive care, understanding symptoms for timely intervention. Reference: Urbach D, et al. "Surgical management of gynecologic emergencies." Journal of Obstetrics and Gynecology, 2020.
