About This Presentation

Description : Understanding Uterine Fibroids: Symptoms, Diagnosis, and Treatment Options
Uterine fibroids, also known as leiomyomas, are benign tumors of the uterus and one of the most common gynecological disorders affecting women of reproductive age. These growths can vary greatly in size and location within the uterus, impacting symptoms and treatment approaches. While some women with fibroids may remain asymptomatic, others experience symptoms such as heavy menstrual bleeding, pelvic pain, frequent urination, and reproductive issues.
This presentation provides a comprehensive overview of uterine fibroids, covering the different types (intramural, submucosal, subserosal, and pedunculated) and potential risk factors, including age, family history, and hormonal influences. Attendees will gain insights into various diagnostic tools, from pelvic exams and ultrasounds to advanced imaging techniques like MRI and hysteroscopy. Treatment options range from lifestyle modifications and medications to surgical interventions, such as myomectomy and hysterectomy, depending on symptom severity and patient needs.
With an emphasis on current research and advancements in minimally invasive treatments, this presentation aims to educate individuals on managing fibroids effectively. Join us to understand uterine fibroids, improve patient care, and promote women’s health and well-being.
Explore this essential guide to uterine fibroids and empower yourself with knowledge for better health decisions.

What are Uterine Fibroids? Uterine fibroids, also known as leiomyomas or myomas, are benign smooth muscle tumors of the uterus. They are the most common type of benign gynecological tumors. Fibroids vary in size, location, and can be asymptomatic or cause a variety of symptoms. Reference: Stewart, E. A. (2015). "Uterine fibroids." The Lancet, 379(9822), 2053-2062.

Types of Uterine Fibroids Intramural: Located within the muscular wall of the uterus. Submucosal: Projects into the uterine cavity. Subserosal: Projects to the outside of the uterus. Pedunculated: Attached to the uterus by a stalk. Reference: Catherino, W. H., et al. (2011). "Classification and reporting systems for leiomyomas." Fertility and Sterility, 96(4), 928-939.

Epidemiology of Uterine Fibroids Uterine fibroids affect up to 70-80% of women by age 50. More prevalent in African American women. Higher incidence among women of reproductive age (late 20s to early 50s). Reference: Baird, D. D., et al. (2003). "High cumulative incidence of uterine leiomyoma in black and white women." American Journal of Obstetrics and Gynecology, 188(1), 100-107.

Causes and Risk Factors Exact cause unknown but believed to be influenced by hormonal (estrogen and progesterone) and genetic factors. Risk Factors: Age (more common in reproductive years) Family history Ethnicity (higher risk in African American women) Early onset of menstruation Protective Factors: Higher parity, late menarche. Reference: Peddada, S. D., et al. (2008). "Growth of uterine leiomyomata among premenopausal black and white women." Proceedings of the National Academy of Sciences, 105(50), 19887-19892.

Symptoms of Uterine Fibroids Common Symptoms: Heavy or prolonged menstrual bleeding Pelvic pain or pressure Frequent urination Constipation Pain during intercourse Many women are asymptomatic, and fibroids may be found incidentally. Reference: Marsh, E. E., et al. (2014). "Epidemiology, health care utilization, and costs of uterine fibroids." Obstetrics and Gynecology Clinics of North America, 41(1), 1-11.

Diagnosis of Uterine Fibroids Pelvic Exam: Initial examination. Ultrasound: Primary imaging tool to visualize fibroids. MRI: Used for detailed imaging, especially pre-surgery. Hysteroscopy: To view fibroids within the uterine cavity. Other Tests: Blood tests to check for anemia due to heavy bleeding. Reference: Stewart, E. A., et al. (2016). "Clinical practice. Uterine fibroids." New England Journal of Medicine, 375(4), 379-388.

Treatment Options Watchful Waiting: For asymptomatic fibroids. Medications: Hormonal therapy (e.g., GnRH agonists) Non-hormonal options (NSAIDs for pain) Non-surgical Options: Uterine artery embolization (UAE) MRI-guided focused ultrasound Surgical Options: Myomectomy (fibroid removal) Hysterectomy (removal of uterus) Reference: Gupta, J. K., et al. (2012). "Uterine artery embolization for symptomatic uterine fibroids." Cochrane Database of Systematic Reviews.
Slide 10-

Risks and Complications Complications of untreated fibroids: Severe anemia Reproductive issues (infertility, pregnancy complications) Urinary or bowel dysfunction Risks of Treatments: Side effects of medications, surgical risks. Reference: Donnez, J., & Dolmans, M. M. (2016). "Uterine fibroid management: from the present to the future." Human Reproduction Update, 22(6), 665-686.
Slide 11-

Prevention and Lifestyle Modifications There is no sure way to prevent fibroids, but lifestyle modifications may reduce risk: Maintain a healthy weight Diet rich in fruits and vegetables Regular exercise Discuss preventive measures for symptom management. Reference: Wise, L. A., et al. (2015). "Dietary fat and risk of uterine leiomyomata." American Journal of Epidemiology, 181(10), 722-733.
Slide 12-

Recent Research and Advances New minimally invasive techniques (e.g., radiofrequency ablation). Advances in pharmacological options targeting fibroid growth. Genetic research identifying specific mutations associated with fibroids. Reference: Bulun, S. E. (2013). "Uterine fibroids." New England Journal of Medicine, 369(14), 1344-1355.

Key Takeaways
Slide 14-

Uterine fibroids are common and often manageable. Treatment should be individualized based on symptoms, size, location, and patient goals. Early detection and lifestyle adjustments can help in managing symptoms.
