Cardiac Devices IMPLANTATION Audit Tgh 2018 Rehman RANA-SPR AAMIR AYUB-CONSULTANT CARDIOLOGIST
Slide 2-

Objectives To quantify the number of pacemaker insertions, generator change and Linq device implantations performed during 2018. To assess local complication rates and compare them with previous years and national rates. To Identify areas for future improvement
Slide 3-

methodology Number of procedures performed between 1/1/2018 and 31/12/2018 obtained from pacemaker logbook and crosschecked with electronic database. Demographics of the operator, indications for the procedure. Complications identified from procedural logbook and NICOR. Data obtained and incorporated into a data collection tool and analyzed Data from 2018 compared with results of previous audits from 2015, 2016 and 2017
Slide 4-

CARDIAC DEVICES IMPLANTED AT Tgh 1. PERMANENT PACEMAKERS • Single chamber • Dual chamber 2. GENERATOR CHANGE 3. TEMPORARY PACING WIRES 4. LINQ DEVICE
Slide 5-

Single Chamber pacemaker • Cheaper • Quicker to implant • AV synchrony may not be maintained • Mainly indicated in patients with AF
Slide 6-

Dual chamber pacemaker • More physiologically accurate • Maintains AV synchrony • Less risk of PPM syndrome • More time consuming to implant • Expensive
Slide 7-

Implantable loop recorder Used to monitor heart rate and rhythm Patients who experience syncope, presyncope. Patients at increased risk of arrhythmia

Procedure trends
Slide 13-

COMPLICATIONS
Slide 14-

outcome Yearly increase in the number of implanted cardiac devices Persistent low complications rate over the years Very low infection rate due to peri and post-operative use of appropriate antibiotics(Teicoplanin and intra-pocket Gentamicin)
Slide 15-

Areas of future developement Allocated pacing suites/theatres Requirement of adequate and more trained staff Continuity of the audit process to look for further improvement in the quality of care and service provision
