About This Presentation

Description : Check out this medical PowerPoint presentation titled "Hysteroscopy”.

This medical PowerPoint presentation is about hysteroscopy, a medical procedure that involves the use of a thin, lighted tube called a hysteroscope to examine the inside of the uterus. The hysteroscope is inserted through the vagina and cervix and into the uterus, allowing the doctor to see the lining of the uterus and the openings of the fallopian tubes.

Hysteroscopy can be used for both diagnostic and therapeutic purposes. In a diagnostic hysteroscopy, the doctor can visualize the uterus and diagnose conditions such as abnormal bleeding, fibroids, polyps, adhesions, or uterine anomalies. In a therapeutic hysteroscopy, the doctor can use the hysteroscope to remove polyps or fibroids, treat adhesions or scar tissue, or perform other surgical procedures.

Hysteroscopy is usually performed on an outpatient basis and can be done with local or general anesthesia. It is generally a safe and effective procedure with minimal risks, but as with any medical procedure, there is a small risk of complications such as bleeding, infection, or perforation of the uterus. Your doctor will discuss the risks and benefits of hysteroscopy with you before the procedure.

Hysteroscopy : passing a small diameter telescope , either flexible or rigid , through the cervix to directly inspect the uterine cavity Flexible hysteroscopy may be used in the out patient setting , with carbon dioxide as a filling medium Rigid instrument employ circulating fluids and can be used to visualze the uterine cavity even if the woman is bleeding

Distention Media Because the anterior and posterior uterine walls lie in apposition, a distention medium is required to expand the endometrial cavity for viewing. Media include carbon dioxide (CO2) Saline low-viscous fluids such as sorbitol, mannitol, and glycine An ordinary manually operated pressure cuff is pumped up so the intrauterine pressures of these media must reach 45 to 80 mm Hg Rarely is more than 100 mm Hg required
Slide 5-

Sorbitol/mannitol and glycine solutions are only used in surgical hysteroscopy, as they are electrolyte-free.

hystroscope This common procedure has specific safety issues: Distension media include co2, normal saline, or glycine Water is not used bec, it is hypo-osmolar and once absorbed causes haemolysis. Uterine perforation may occur; high index of suspicion is needed, with early resource to laparoscopy. Excessive fluid absorption may lead to seizures and even death. Accurate measurement of fluid deficit is needed throughout the procedure. Fluid absorption increases significantly when intrauterine pr. Exceed the main arterial pr or when large vessel open through out endometrial resection surgery

Indications of hysteroscopy 1.Diagnostic hysteroscopy: with target biopsy abnormal menstruation, intermenstrual bleeding, postcoital and postmenopausal bleeding. Abnormal pelvic ultrasound findings (e.g. endometrial polyps, submucous fibroids) Subfertility Recurrent miscarriage Asherman’s syndrome Congenital uterine anomaly g. Lost intrauterine contraceptive device (IUCD)

2.Operative hysteroscopy: Hysteroscopic surgery has a number of well-defined indications and is the treatment of choice for: 1. polypectomy. 2. myomectomy for intracavitary or submucous fibroids 3. adhesiolysis of intrauterine adhesions 4. metroplasty (division/resection of uterine septum). 5.endometrial ablation (destruction of the endometrium). 6. hysteroscopic sterilization
Slide 18-

Contraindications of hysteroscopy 1. Pelvic infection 2.Pregnancy 3.Cervical cancer 4. Heavy uterine bleeding
Slide 19-

Complications of hysteroscopy: Diagnostic hysteroscopy is a safe procedure, and complications are uncommon. However operative hysteroscopy carries a higher risk of complications: 1. Anaesthetic complications 2. Uterine perforation 3. Fluid overload so careful monitoring of fluid input and output is required 4. Haemorrhage 5. Gas embolism 6. Infection 7. Cervical trauma
