Surgical Approaches in Gynecologic Oncology Advanced Surgical Techniques for Gynecologic Cancer Management Dr. Vineeta Bose Oncologist & Hematologist
Slide 2-

Why Surgery Matters in Gynecologic Oncology Surgery is a cornerstone in the treatment of gynecologic cancers. Goals of surgery: Tumor removal. Accurate staging. Symptom relief in advanced cases. Types of cancers addressed: Cervical, ovarian, uterine, vulvar, and vaginal cancers. Speaker Notes: Surgery plays a vital role in both curative and palliative settings for gynecologic cancers. It’s often the first step in cancer management for accurate diagnosis and treatment planning. Reference: Goff, B. A., & Muntz, H. G. (2005). Surgical techniques in gynecologic oncology. Journal of Clinical Oncology, 23(24), 6071-6078.

Radical Hysterectomy in Cervical Cancer Removal of the uterus, upper vagina, and surrounding tissues. Indications: Early-stage cervical cancer. Types: Open abdominal hysterectomy. Laparoscopic or robotic-assisted hysterectomy. Benefits: High cure rates in localized disease. Speaker Notes: Radical hysterectomy remains a primary treatment for early-stage cervical cancer, particularly in women who don’t require fertility preservation. Reference: Benedetti-Panici, P., et al. (1997). Early cervical carcinoma. Cancer, 80(6), 1234-1241.

Cytoreductive Surgery in Ovarian Cancer Goal: Remove as much tumor as possible (debulking). Indication: Advanced-stage ovarian cancer. Procedure: Removal of ovaries, uterus, omentum, and visible tumor deposits. Improves survival when combined with chemotherapy. Speaker Notes: Cytoreductive surgery aims to reduce tumor burden, allowing chemotherapy to work more effectively, especially in advanced ovarian cancer. Reference: Vergote, I., et al. (2010). Optimal cytoreduction in ovarian cancer. New England Journal of Medicine, 363(10), 943-953.

Fertility-Sparing Options in Gynecologic Oncology Radical Trachelectomy: Indication: Early-stage cervical cancer. Removal of the cervix while preserving the uterus. Ovarian Cystectomy: Indication: Early-stage ovarian cancer. Preserves the unaffected ovary and uterus. Importance of counseling patients on risks and outcomes. Speaker Notes: Fertility-sparing surgeries are crucial for younger patients desiring future pregnancies while managing cancer effectively. Reference: Pareja, R., et al. (2013). Radical trachelectomy. Gynecologic Oncology, 131(3), 613-620.

Minimally Invasive Techniques in Gynecologic Oncology Laparoscopy and robotic-assisted surgery. Benefits: Smaller incisions. Reduced blood loss and pain. Faster recovery and shorter hospital stay. Indications: Endometrial cancer staging. Early-stage cervical and ovarian cancers. Speaker Notes: Minimally invasive techniques are transforming gynecologic oncology, offering similar outcomes to open surgeries with fewer complications. Reference: Fader, A. N., & Boruta, D. M. (2017). Minimally invasive surgery in gynecologic oncology. Journal of Gynecologic Oncology, 28(1), e1.

Pelvic Exenteration for Advanced Cancer Indication: Recurrent or advanced gynecologic cancers. Procedure: Removal of pelvic organs (bladder, rectum, reproductive organs). Types: Anterior, posterior, or total exenteration. Challenges: High morbidity but can offer curative potential in select cases. Speaker Notes: Though aggressive, pelvic exenteration remains an option for patients with limited but curable recurrent gynecologic cancers. Reference: Shingleton, H. M., et al. (1995). Pelvic exenteration. Gynecologic Oncology, 57(3), 324-330.

Sentinel Lymph Node Mapping Minimally invasive technique to identify and biopsy sentinel nodes. Indications: Endometrial and cervical cancer staging. Advantages: Reduces complications associated with full lymph node dissection. Accurate staging tool. Speaker Notes: Sentinel lymph node mapping is a game-changer in reducing surgical morbidity without compromising cancer staging accuracy. Reference: Geppert, B., & Lönnerfors, C. (2017). Sentinel lymph node mapping in gynecologic oncology. European Journal of Obstetrics & Gynecology and Reproductive Biology, 211, 125-130.

Advances in Surgical Technology Robotic-assisted platforms for precision surgeries. Use of augmented reality for surgical planning. Integration of AI for predictive outcomes. Enhanced recovery protocols post-surgery. Speaker Notes: Surgical innovation continues to evolve, improving outcomes and patient experiences in gynecologic oncology. Reference: Nezhat, C., et al. (2021). Advances in gynecologic oncology surgery. Current Opinion in Obstetrics & Gynecology, 33(3), 232-238.

Conclusion and Future Directions Surgical approaches remain vital for diagnosis, staging, and treatment. Emphasis on personalized care: Fertility-sparing techniques and minimally invasive surgeries. Future outlook: Technological advancements to further improve outcomes. Speaker Notes: With advancements in surgical techniques and technology, gynecologic oncology continues to improve survival and quality of life for patients.
