What is RAMSAY HUNT Syndrome? Ramsay Hunt syndrome is also known as Hunt syndrome and HERPES ZOSTER Oticus It is a rare neurological disorder charachterised by paralysis of facial nerve and a rash affecting the ear or mouth caused by HERPES ZOSTER VIRUS INFECTION of the Geniculate ganglion of the facial nerve

SIGNS AND SYMPTOMS… THE 2 MAIN SIGNS AND SYMPTOMS OF RAMSAY HUNT SYNDROME; Painful red rash with fluid blisters on ,in, around the ear Facial weakness or paralysis on the same side as the effected ear Usually the rash and facial paralysis occur at same time but sometimes one can happen before the other OTHER SYMPTOMS Ear pain Hearing loss Ringing In ur ears (TINNITUS) Sensation of spinning or moving (VERTIGO ) Change in taste perception or loss of taste Dry mouth and eyes

FUNCTIONAL ANATOMY THE NEURO ANATOMY INVOLVED IN RAMSAY HUNT SYNDROME There are 4 cranial nerve involved in facial nerve functions The motor nucleus of VII CRANIAL NERVE (facial nerve) Nucleus of the solitary tract The superior salivatory nucleus Spinal nucleus of cranial nerve V (Trigeminal nerve)

Motor nuclei of 7th cranial nerve Special visceral efferent motor fibres from the motor nucleus of the 7th cranial nerve leave the brain stem and then travel through the internal acoustic meatus to the bony facial canal to supply the facial muscles IN RAMSAY HUNT SYNDROME these fibres are affected as they pass through the geniculate ganglion thus imparing the motor supply of the facial nerve

Nucleus of the solitary tract The solitary tract recieves special visceral afferent taste fibres fron ant. 2/3 of tongue These fibres travel with the corda tympani The cell bodies of these special visceral afferent fibres are in the geniculate ganglion which is the site of reactivation when vescicles erupt on the tongue The fibres reach the the brain stem via nevus intermedius can be affected by the local inflammation as they pass the geniculate ganglion …

Diff B/W bells palsy and Ramsay Hunt Syndrome…… BELLS PALSY Injury to the facial nerve by the virus infection ,but the suspected cause is unknown No red rash associated with bells palsy Less painful RAMSAY HUNT SYNDROME It is caused by VARICELA ZOSTER (herpes zoster) that also causes chicken pox and shingles RASH present RHS is more painful

Diagnosis .. To diagnose Ramsay Hunt SYNDROME In children –ELISA ,Serum anti-varicella zoster virus Ig G ,IgM ANTBODY TITERS are recommended Occasionally nerve conduction studies done to determine the extend of nerve damage of facial nerve Structural lesions are ruled out by CT scan .MRI ,MAGNETIC RESONANCE ANGIOGRAPHY

Prevention.. VACCINATION IS RECOMMENDED FOR ALL THE PATIENTS OLDER THAN 60 YRS IF THEY HAD CHICKEN POX OR ZOSTER IN the past CONTRINDICATIONS: Younger than 60yrs Current use of antivirals Pregnancy Immunosuppressive conditions
