Overview of Medical Oncology Understanding the Role of Systemic Cancer Therapy in Modern Medicine Dr. Vineeta Bose Oncologist & Hematologist Medical Oncology
Slide 2-

What is Medical Oncology? A branch of medicine focused on the diagnosis, treatment, and management of cancer using systemic therapies. Systemic therapies include chemotherapy, immunotherapy, targeted therapy, and hormonal therapy. Core goal: Improve survival rates and quality of life for cancer patients. Speaker Notes: Medical oncology is an integral part of multidisciplinary cancer care, working alongside surgical and radiation oncology to offer comprehensive cancer management. Reference: National Cancer Institute. (2021). What is Medical Oncology? Retrieved from https://www.cancer.gov

Historical Perspective 1940s: Introduction of chemotherapy (e.g., nitrogen mustard for lymphoma). 1980s: Development of hormonal therapies for breast cancer. 2000s: Advances in targeted therapy (e.g., trastuzumab for HER2-positive breast cancer). 2010s: Rise of immunotherapy (e.g., checkpoint inhibitors like nivolumab). Speaker Notes: The field of medical oncology has evolved dramatically, with newer therapies offering better outcomes and fewer side effects. Reference: DeVita, V. T., Hellman, S., & Rosenberg, S. A. Role of Medical Oncologists Assessing cancer stage and type. Developing and administering systemic treatment plans. Monitoring patient response and managing side effects. Providing supportive care and counseling. Participating in cancer research and clinical trials. Speaker Notes: Medical oncologists act as coordinators of cancer care, ensuring patients receive tailored treatments and comprehensive support. Reference: World Health Organization. (2021). Types of Systemic Therapies Chemotherapy: Targets rapidly dividing cancer cells. Targeted Therapy: Focuses on specific molecules driving cancer growth (e.g., HER2, EGFR). Immunotherapy: Boosts the immune system to attack cancer cells. Hormonal Therapy: Blocks hormones like estrogen or testosterone that fuel certain cancers. Speaker Notes: These therapies are often used alone or in combination to maximize effectiveness and minimize resistance. Reference: American Society of Clinical Oncology (ASCO). (2021). Multidisciplinary Approach Collaboration with: Surgical Oncologists: For tumor removal and biopsies. Radiation Oncologists: For combining systemic therapy with localized radiation. Pathologists and Radiologists: For accurate diagnosis and treatment planning. Importance of tumor boards in decision-making. Speaker Notes: Medical oncology thrives in collaboration, ensuring comprehensive and personalized cancer care. Reference: Gellhorn, A. C., & Sundararajan, V. (2018). Advances in Medical Oncology Precision medicine: Treatments tailored to the patient’s genetic profile. Liquid biopsies: Non-invasive methods for monitoring cancer progression. Antibody-drug conjugates (ADCs): Targeted drug delivery to cancer cells. CAR-T cell therapy: Genetically engineered immune cells for cancer treatment. Speaker Notes: Advances in technology and research have paved the way for more precise and less toxic treatments, improving outcomes for patients. Reference: Maemondo, M., et al. (2020). Advances in Oncology Drug Development. The Lancet Oncology, 21(4), 487-497.

Challenges in Medical Oncology Drug resistance: Tumors adapting to therapies. Managing side effects: Nausea, fatigue, immune-related adverse events. Financial toxicity: High cost of systemic therapies. Addressing disparities: Access to care and equity in treatment delivery. Speaker Notes: While progress is remarkable, medical oncology still faces challenges, especially in ensuring equitable care for all patients. Reference: Kantarjian, H., et al. (2019). Financial Barriers in Cancer Care. JAMA Oncology, 5(6), 817-824.
