About This Presentation

Description : NHL are a heterogeneous group of disorders involving malignant monoclonal proliferation of lymphoid cells in lymphoreticular sites, including lymph nodes, bone marrow, the spleen, the liver, and the GI tract. NHL is more common than Hodgkin lymphoma. This Incidence increases with age (median age, 50 years).

CASE STUDY Jason is a 42-year-old male patient who has been admitted to the hospital with abdominal distension and dyspnea that has developed over the past 4 weeks. Jason also reports having night sweats and states that he has become increasingly tired and lost about 10 lbs. in the past month. He reports poor appetite and exercise intolerance. During examination he is noted to have decreased breath sounds in the right lung base and ascites is also noted. A 3 cm lymph node is noted in the left inguinal region and enlarged cervical lymph nodes, all painless.

ETIOLOGY NHL cause is unknown, but substantial evidence suggests a viral cause HIV Epstein-Barr Virus Hepatitis C

RISK FACTORS
Slide 6-

PATHOPHYSIOLOGY 80-85% of NHL arise from B lymphocytes 15-20 % on NHL arise from T lymphocytes or NK cells. Most lymphomas are nodal with variable involvement of the bone marrow and peripheral blood. Leukemia-like picture with peripheral lymphocytosis and bone marrow involvement, more common in children (50%) than adults (20%)
Slide 7-

HODGKIN VS NON-HODGKIN Hodgkin lymphoma: Lymphoma arises in a single node or chain of nodes. Reed-Sternberg cells are present Non-Hodgkin lymphoma: Lymphoma originates at extranodal sites and spreads to anatomically contiguous nodes. Reed-Sternberg cells are not present Normal Lymphocyte (NHL) Hodgkin Lymphoma

DIAGNOSIS
Slide 12-

STAGING Categories: E - Stands for ”Extranodal” S – Stands for ”Spleen” X – Indicates “Bulky disease” Example: lllE+S: Involvement of lymph node groups above and below the diaphragm, outside the lymph nodes and in a nearby organ or area, and in the spleen.

TREATMENT
Slide 14-

PROGNOSIS Chemotherapy with or without radiation achieves cure of 70-80% Disease-free survival 5 year after therapy is considered a cure. Relapse is very rare after 5 years. Patients who do not achieve complete remission or who relapse within 12 months have a poor prognosis
