IHI Triple Aim for Populations Lowering Costs of Care
Slide 2-

Even more than just lowering cost we need to think of how to increase value!
Slide 3-

Money Saved can be used to improve the quality of health care can be used to improve the overall health of populations
Slide 4-

A lot of the wasteful spending on healthcare is from health issues that are treated in an acute setting but they could be more easily and less expensively treated with primary care or community care services.
Slide 5-

We cannot lower costs simply by reducing quality…to achieve the Triple Aim…
Slide 6-

We need to increase Value!Value = Quality / Cost
Slide 7-

Using this formula you can increase the value either by increasing quality or by decreasing cost
Slide 8-

On top of the extra cost of emergency room visits, there is also an increased risk for miscommunication which can lead to medical errors
Slide 9-

Population Managementwill also help to reduce costs, but will require health professionals to work across disciplines and across communities
Slide 10-

Another way to improve quality and reduce cost is to have intensive follow up programs - both from Acute care settings and primary care settings
Slide 11-

Follow up could enhance better patient outcomes, decrease readmittance, provide intervention before acute care is needed, and increase on time preventive screenings
Slide 12-

And at the time of follow up we could help to solve “social” problems that are interfering with patient compliance!
Slide 13-

do not forget resource stewardship:The appropriate allocation of resources
Slide 14-

Every single person in healthcare needs to embrace this concept and do their part to ensure money is not wasted on unnecessary testing, equipment, and medications!
