Hepatic Encephalopathy It’s a spectrum of neuropsychiatric abnormalities in patients with liver dysfunction, after exclusion of brain disease. Hepatic encephalopathy is observed in patients with cirrhosis and also described in patients without cirrhosis with either spontaneous or surgically created portosystemic shunts. 3

A nomenclature has been proposed for categorizing hepatic encephalopathy: Type A: hepatic encephalopathy describes encephalopathy associated with A cute liver failure. Type B: hepatic encephalopathy describes encephalopathy associated with portal-systemic Bypass and no intrinsic hepatocellular disease. Type C: hepatic encephalopathy describes encephalopathy associated with Cirrhosis and portal hypertension or portal-systemic shunts. Type C hepatic encephalopathy is, in turn, subcategorized as episodic, persistent, or minimal. 4

Epidemiology: The exact incidence of hepatic encephalopathy in the general population is unknown. It affects males and females in equal numbers and can occur in individuals of any age who have acute or chronic liver disease. Approximately 24-53 % of individuals whose liver is bypassed by a portosystemic shunt (with no liver disease present) develop hepatic encephalopathy. Approximately 70 % of individuals with cirrhosis may develop symptoms of hepatic encephalopathy. Because some mild cases of hepatic encephalopathy may go undiagnosed, it is difficult to determine the true frequency in the general population. 5

Pathogenesis A number of theories have been proposed to explain the development of hepatic encephalopathy in patients with cirrhosis. Some investigators contend that hepatic encephalopathy is a disorder of astrocyte function. Astrocytes account for about one third of the cortical volume. They play a key role in: Regulation of the blood-brain barrier. Maintaining electrolyte homeostasis . Providing nutrients and neurotransmitter precursors to neurons. Detoxification of a number of chemicals, including ammonia. 7

Ammonia hypothesis Normally, ammonia is detoxified in the liver by conversion to urea by the Krebs-Henseleit cycle. Ammonia is also consumed in the conversion of glutamate to glutamine, a reaction that depends upon the activity of glutamine synthetase. Two factors contribute to the hyperammonemia that is seen in cirrhosis: 1) The decrease in the mass of functioning hepatocytes, resulting in fewer opportunities for ammonia to be detoxified by the above processes. 2) Portosystemic shunting may divert ammonia-containing blood away from the liver to the systemic circulation. Ammonia has multiple neurotoxic effects: 1)Alter the transit of amino acids, water, and electrolytes across astrocytes and neurons. 2)Impair amino acid metabolism and energy utilization in the brain. 8

GABA Hypothesis GABA is a neuroinhibitory substance produced in the gastrointestinal tract. Of all brain nerve endings, 24-45% may be GABAergic. For over 20 years, it was postulated that hepatic encephalopathy was the result of increased GABAergic tone in the brain. However, experimental work is changing perceptions regarding the activity of the GABA receptor complex in cirrhosis. Neurotoxins, like ammonia and manganese, increase the production of the peripheral-type benzodiazepine receptor (PTBR) in astrocytes. PTBR, in turn, stimulates the conversion of cholesterol to pregnenolone to neurosteroids. Neurosteroids are then released from the astrocytes. They are capable of binding to their receptor within the neuronal GABA receptor complex and can increase inhibitory neurotransmission. 9

Diagnosis Hepatic encephalopathy is a diagnosis of exclusion of other causes of altered mental status with exploring precipitating factors for hepatic encephalopathy. Diagnostic tests include: Laboratory Testing . Imaging. Neuropsychometric Tests. 12

Laboratory Testing arterial and venous ammonia levels: There are many non-hepatic causes of hyperammonemia, such as gastrointestinal bleeding, renal failure, hypovolemia, extensive muscle use, urea cycle disorder, parenteral nutrition, urosepsis, and with use of certain drugs (e.g. valproic acid). Although patients with hepatic encephalopathy have elevated serum ammonia levels, the severity of hepatic encephalopathy does not correlate with serum ammonia levels beyond a certain point. Moreover, serial ammonia levels are not routinely utilized to follow patients, as the clinical presentation and clinical response to treatment is most important. Other laboratory tests : to assess for precipitating causes of hepatic encephalopathy, such as tests of liver and renal function, electrolytes, glucose, cultures, and drug screening. 13

Imaging Brain computed tomographic (CT) imaging : low sensitivity for detecting early cerebral edema but may help to exclude other causes of altered mentation, such as an intracerebral hemorrhage. Brain magnetic resonance imaging (MRI): can be used to diagnose cerebral edema and other brain abnormalities associated with hepatic encephalopathy. Other types of imaging can also be used to assess for hepatic encephalopathy precipitating factors, such as infection (e.g. chest radiograph) or bowel obstruction or ileus (e.g. abdominal imaging). 14

Neuropsychometric Tests neuropsychometric tests include both ‘paper and pencil’ tests and computerized tests that can be used to identify impairments in visuo-spatial functioning, attention, processing speed and response inhibition. These tests are helpful in identifying minimal hepatic encephalopathy. The Psychometric Hepatic Encephalopathy Score (PHES) is the current ‘gold standard’ pencil-and-paper test battery for the psychometric evaluation of patients with HE. 15

Management of Hepatic Encephalopathy the management of acute hepatic encephalopathy should focus on : providing supportive care, identifying and treating any precipitating causes, reducing nitrogenous load in the gut, and assessing the need for long term therapy and liver transplant evaluation. Common precipitating factors for HE include: Increased nitrogen load (e.g. gastrointestinal bleed, infection, excess dietary protein). Decreased toxin clearance (e.g. hypovolemia, renal failure, constipation, portosystemic shunt, medication noncompliance, acute on chronic liver failure), Altered neurotransmission (e.g. sedating medication, alcohol, hypoxia, hypoglycemia). 17

Non absorbable Disaccharides “lactulose” first-line treatment for hepatic encephalopathy. Lactulose is metabolized by bacteria in the colon to acetic and lactic acid which: reduces colonic pH. decreases survival of urease producing bacteria in the gut. facilitates conversion of ammonia (NH3) to ammonium (NH4+) “↓absorption” The cathartic effect also increases fecal nitrogen waste. 19

Non absorbable Disaccharides “lactulose” For acute overt hepatic encephalopathy, the usual starting oral dose of lactulose is 10 to 30 g (15 to 45 mL) every 1 to 2 hours until a bowel movement occurs. Then adjust to 10 to 30 g (15 to 45 mL) 2 to 4 times daily, titrated to induce 2 to 3 soft bowel movements daily. For comatose patients, the medication can be administered through a nasogastric tube or rectally as an enema (300 mL in 1 L of water every 6 to 8 hours) until the patient is awake enough to start oral therapy. Side effects can include an excessively sweet taste, abdominal cramping, flatulence, and diarrhea. Severe cases of diarrhea can lead to hypovolemia and electrolyte imbalances. 20

Antibiotics Rifaximin is a minimally absorbed (less than 0.4%) oral antibiotic with broad spectrum activity against gram-positive and gram-negative aerobic and anaerobic bacteria. Rifaximin (550 mg twice daily) can be used to treat hepatic encephalopathy. rifaximin with lactulose maintained remission from hepatic encephalopathy better than lactulose alone. In clinical practice, lactulose is typically used initially and rifaximin added if needed. 21

Nutrition Dietary protein restriction is not advised for the management of hepatic encephalopathy since loss of skeletal muscle, which metabolizes ammonia, can lead to worsening hepatic encephalopathy. patients with cirrhosis are recommended to consume a high-protein diet of at least 1.0 g/kg to 1.5 g/kg daily. Non-absorbable vegetable fiber can help promote colonic acidification. Zinc is a cofactor for urea cycle enzymes and is commonly deficient in cirrhotic patients. 22
