Particulars of the patient: Name: Moniruzzaman Age: 43 years Sex: male Occupation: shopkeeper Hand dominance: right Address: Shirazdikhan, Munshigonj Date of admission: 14-11-22 Date of Examination: 18-11-2022

Chief Complaints RTA followed by pain and swelling over right forearm for 2 weeks Difficulty in movement of right elbow and wrist for 2 weeks

History of present illness According to the statement if the patient he was reasonably well 2 weeks back. Then he Had a RTA where he was hitted by an autorickshaw. Immediately he developed severe pain and swelling over his right forearm and he had difficulty in movement of right elbow and wrist. The pain was aggravated on movement of the limb. He had no injury in any other part of the body. He went to NITOR and LABS was applied. He had no other constitutional symptoms. With these complaints he was admitted in BSMMU for further management.

General examination Appearance: Anxious Body built: Average Anaemia: Absent Jaundice: Absent Dehydration : Absent Oedema : Absent Temperature: 98.5°F Pulse: 88 /min BP: 120/70mmhg Respiratory rate : 12/min Lymphnodes : not palpable Systemic examination No abnormality

Loco-regional examination After careful removal of back slab Look: Skin: pigmentation over the ulnar aspect of right midforearm No scar, bruise or laceration. Apparent muscle wasting over right forearm Mild radial deviation of right forearm at midforearm.

Locoregional examination Feel: Temp: normal Tenderness: over the lateral aspect of right midforearm. Muscle wasting: about 1 cm wasting over right forearm. Distal neurovascular status: intact

Locoregional examination Move: Range of movement over right elbow and wrist couldn’t be elicited due to pain. Range of movement of shoulder and hand joints – Normal
