Our case is talking about a man with blurry vision. Describe the intra ocular fluid responsible for maintaining of IOP? Discuss how it is secreted and drained? Q1:
Slide 4-

Aqueous humor Alkaline clear transparent fluid. Its Volume is 200 mm3. Fill both anterior and posterior chambers. Carries nutrients to the non vascular structures of the eye like cornea and lens. Contains sodium chloride, small amount of protein reducing sugar and hyaluronic acid which kept the depolymerized state by hyaluronidase enzyme. Contains amino acids, ascorbic acid and HCO3. Keeps IOP constant (20mm/Hg). Gives the eye its globular shape.
Slide 5-

Aqueous humor formation The ciliary epithelium secretes actively sodium ions into the aqueous humor → both Cl and HCO3 ions are probably pulled passively along with the Na by the positive charges of the sodium → increase the osmotic pressure of the aqueous humor → osmosis of large amounts of water through the ciliary epithelium to inside of the eye.
Slide 6-

Aqueous humor formation The active secretion of aqueous humor is proved by: A) The concentration of Na ions and Cl ions are higher in the aqueous humor than in the plasma. B) The osmotic pressure of the aqueous humor is 3-5 milliosmoles greater than the plasma. Such difference promotes adequate flow of water from the plasma to inside the eye across the ciliary membrane. C) The concentration of glucose and urea is lower than the plasma. D) Direct studies of the epithelial membrane of the ciliary process using isotopic study have demonstrated active transport of Na ions from the plasma into the aqueous humor.
Slide 7-

Aqueous humor drainage The aqueous humor is formed by the ciliary processes and enters the posterior chamber → flows through the suspensory ligament of the lens → through the pupil → anterior chamber of the eye → fluid flows into the angle between the cornea and iris called iridocorneal or filtration angle → spaces between a network of trabeculae called spaces of Fontana → canal of Schlemm(The wall of this canal is thin and extremely permeable even to large protein molecules) → aqueous veins → ocular veins.
