Introduction to Endometriosis and Pelvic Pain Endometriosis is a chronic condition where tissue similar to the lining of the uterus grows outside the uterus. Affects up to 10% of women of reproductive age globally. Often associated with chronic pelvic pain, menstrual irregularities, and fertility issues. Speaker Notes: Endometriosis is a common but often underdiagnosed condition that can have a profound impact on a woman’s quality of life, including chronic pain and challenges with fertility. Reference: Giudice, L. C., & Kao, L. C. (2004). Endometriosis. The Lancet, 364(9447), 1789-1799.

Symptoms of Endometriosis and Pelvic Pain Chronic pelvic pain, particularly during menstruation (dysmenorrhea). Pain during sexual intercourse (dyspareunia). Pain with bowel movements or urination. Heavy menstrual bleeding or bleeding between periods. Infertility in some women. Speaker Notes: Symptoms vary widely among women with endometriosis. While some may have severe pain, others may have few symptoms despite extensive endometriosis. Reference: Bulun, S. E. (2009). Endometriosis. New England Journal of Medicine, 360(3), 268-279.

Causes and Risk Factors Retrograde menstruation theory: Menstrual blood flows back into the pelvis. Genetic predisposition. Hormonal imbalances (estrogen dominance). Immune system dysfunction. Risk factors: Early menstruation, short menstrual cycles, family history. Speaker Notes: While the exact cause of endometriosis is unknown, several theories exist. Retrograde menstruation is the most widely accepted theory but does not explain all cases. Reference: Missmer, S. A., & Cramer, D. W. (2003). The epidemiology of endometriosis. Obstetrics & Gynecology Clinics, 30(1), 1-19.

Diagnosing Endometriosis Patient history and physical exam. Imaging tests: Ultrasound: Often the first imaging test, can identify endometriomas. MRI: Provides more detail on endometriosis location. Laparoscopy: Gold standard for diagnosis; allows direct visualization and biopsy. Speaker Notes: Diagnosing endometriosis can be challenging, as symptoms overlap with other conditions. Laparoscopy is the definitive way to diagnose, but non-invasive imaging is often used initially. Reference: Kennedy, S., Bergqvist, A., Chapron, C., D’Hooghe, T., Dunselman, G., & Saridogan, E. (2005). ESHRE guideline for the diagnosis and treatment of endometriosis. Human Reproduction, 20(10), 2698-2704.

Impact of Endometriosis on Reproductive Health Infertility affects 30-50% of women with endometriosis. Possible causes: Inflammatory environment affecting the reproductive organs. Adhesions leading to structural changes. Impaired implantation of the embryo. Speaker Notes: Endometriosis can significantly impact fertility due to inflammation and structural changes in the reproductive organs, making it more difficult for an embryo to implant. Reference: Zondervan, K. T., Becker, C. M., & Missmer, S. A. (2020). Endometriosis. New England Journal of Medicine, 382(13), 1244-1256.

Treatment Options Medications: NSAIDs for pain relief. Hormonal therapy (oral contraceptives, GnRH agonists). Surgery: Laparoscopic excision of endometrial implants. Hysterectomy in severe cases. Alternative Therapies: Physical therapy, acupuncture, and dietary changes. Speaker Notes: Treatment depends on the severity of symptoms and fertility desires. Hormonal therapies are often first-line, while surgery is considered for severe cases. Reference: Vercellini, P., Somigliana, E., Viganò, P., Abbiati, A., & Fedele, L. (2009). Endometriosis: Current therapies and new pharmacological developments. Drugs, 69(6), 649-675.
Slide 9-

Pain Management in Endometriosis Lifestyle modifications (diet, exercise). Physiotherapy for pelvic pain. Cognitive-behavioral therapy (CBT) for coping with chronic pain. Multidisciplinary pain management approach. Speaker Notes: Effective pain management in endometriosis often requires a multidisciplinary approach. Lifestyle changes and cognitive therapy can play a significant role in managing chronic pain. Reference: Vitale, S. G., La Rosa, V. L., Rapisarda, A. M. C., & Laganà, A. S. (2017). Slide 10-

Psychosocial Impact of Endometriosis Emotional and mental health effects (anxiety, depression). Impact on intimate relationships and social life. Reduced quality of life due to chronic pain and infertility concerns. Speaker Notes: The psychosocial effects of endometriosis are significant, with many women experiencing emotional challenges. Support from healthcare professionals can help patients cope. Reference: De Graaff, A. A., D'Hooghe, T. M., Dunselman, G. A., Dirksen, C. D., Hummelshoj, L., & Simoens, S. (2013). Slide 11-

Supporting Women with Endometriosis Providing education about the condition and treatment options. Encouraging support groups and mental health resources. Ensuring follow-up care to address ongoing symptoms. Building a multidisciplinary care team for comprehensive support. Speaker Notes: A holistic approach to support is essential for managing endometriosis. Education, support groups, and mental health resources can empower patients and improve quality of life. Reference: Parasar, P., Ozcan, P., & Terry, K. L. (2017). Endometriosis: Epidemiology, diagnosis, and clinical management. Current Obstetrics and Gynecology Reports, 6(1), 34-41.
