Classification
Slide 5-

Signs And Symptoms Fatigability Eye and eyelid muscles a) Especially susceptible Facial expression muscles Chewing muscles Talking muscles Swallowing muscles Breathing muscles b) Can also be affected Neck and limbs muscles
Slide 6-

Signs And Symptoms (cont.) The first symptom : a) In most cases: weakness of eye muscle b) In others: difficulty in swallowing and slurred speech Asymmetrical ptosis Diplopia Unstable or waddling gate
Slide 7-

Signs And Symptoms (cont.) Weakness in arms, hands, etc. Change in facial expression Dysphagia Shortness of breath Dysarthria Paralysis of the respiratory muscles in myasthenic crisis
Slide 8-

Pathophysiology Autoimmune channelopathy No relation to cross-reaction Antibodies' acting against nAchR: a) Impairing the ability of acetylcholine to bind to receptors b) Destruction of receptors Complement fixation Elimination of the receptors Antibodies acting against MuSK proteins : Decreasing in patency of the NMJ

Diagnosis Physical examination Blood test Electrodiagnostics
Slide 12-

Diagnosis-Physical Examination Looking upward and sideward for 30 seconds: ptosis and diplopia Looking at the feet while lying on the back for 60 seconds Keeping the arms stretched forward for 60 seconds Ten deep knee bends Walking 30 steps on both the toes and the heels Five sit-ups, lying down and sitting up completely
Slide 13-

Diagnosis-Blood Test Identifying certain antibodies: a)Ab against AchR B)Ab against MuSK protein
Slide 14-

Diagnosis-Electrodiagnostics Mal-response of muscles to repeated stimulation Measurement of muscle fatigability
