1. Scapula : Abduction and upward rotation Grade 5 : Scapula maintains its abducted and rotated position against maximal resistance given on the arm just above the elbow in a downward direction. Grade 4 : Scapular muscles "give" or "yield“ against maximal resistance given on the arm. The glenohumeral joint is held rigidly in the presence of a strong deltoid, but the serratus yields, and the scapula moves in the direction of adduction and downward rotation. Scapula : Abduction and upward rotation Grade 2 : If the scapula abducts and rotates upward as the patient attempts to hold the arm in the elevated position, the weakness is in the glenohumeral muscles. The serratus is awarded a grade of 2. The serratus is graded 2- (Poor-) if the scapula does not smoothly abduct and upwardly rotate without the weight of the arm or if the scapula moves toward the vertebral spine.
