CASE Our patient is an 83-year old with a history of major depressive disorder, hypertension, and atrial fibrillation. She lives at home with her husband. Depression was previously well controlled on sertraline 200mg qday, but she has not taken it for about 5 years because of symptom improvement. She describes a return of depression symptoms over the prior 4 months.

Case In addition to usual depression symptoms our patient reports the recent onset of memory difficulty. She said she has a hard time concentrating and remembering recent events as well as events that took place years ago. Her son confirms. He says she gets very upset over her forgetfulness and answers, “I don’t know,” to most of his questions. There are no difficulties with IADLs. On brief cognitive testing she is fully oriented. With considerable encouragement she participates in tests of attention and verbal memory which are within normal limits.
