procedure Always get your policies and procedures Remember that it should be a sterile procedure place the patient in semi-fowlers position Suction the patient to remove secretion Change gloves put on sterile gloves Saturate a sterile gauze with solution squeeze out excess and wipe around the patient’s neck under ties Saturate a second gauze continue cleaning the skin around the trach until all clean wipe only once with each gasuze Dry the area and change the dressing

9. Sterile gloves 10. Unlock the tracheostomy by turning counterclockwise 11. Place the inner cannula in a container of peroxide 12. Quickly scrub the cannula with the sterile nylon brush 13.If you are having trouble getting brush in use pipe cleaners After clean place the cannula in normal saline and agitate for 10 seconds Inspect and repeat cleaning process if necessary When clean tap on the side of the container to remove excess moisture Do not dry the outside, replace the inner cannula and change dresing Procedure continued- Cleaning an inner cannula

This is a two person job Put on sterile gloves Use commercially prepackaged ties Have your assistant hold tracheostomy in place Take off the new ties Thread one end the tie through one eye of tracheostomy tube Changing tracheostomy ties

Obtain order Obtain policies and procedures Assess the patient’s vital signs Wash your hand and put on personal protective equipment Explain the procedure to the patient Place the patient in semi-fowlers or high fowlers Open the top of sterile water bottle Open the package containing sterile solution container Using aseptioc technique open suction cathter kit, put non sterile glove on non dominant hand and sterile glove on dominant hand Using non dominant hand pour NS into container Suctioning Procedure

11. Open lubricant for catheter 12. Use your sterile hand to open catheter and remove remember to coil so that tubing does not touch anything 13. Dip the catheter in the normal saline to lubricate the inside 14. Lubricate with water soluble lubricant Preoxygenate the patient Disconnect the oxygen form your patient Use you non dominant hand insert the catheter into the patient’s nostril Advance the catheter as far as possible Apply suction intermittenly by closing and opening control valve with your thumb Suctioning continued
