Case Scenario 48 years old military personnel , posting as a trainer Presented with acute on top of chronic intense right heel pain. He had a trauma while running in a training session 18 month back and his foot was not the same anymore; heel pain was a somehow manageable problem that show up every now and then; despite using a proper training shoes and insoles Pain got intense over the last week prior to presentation after a training session that involved running for a long distance At this time the only relieving factor is just rest, which is inconvenient in his line of work as well as to himself as he love exercising

Case Scenario Upon physical examination the right foot revealed a normal longitudinal arch in the non-weight bearing position. The arch flattens considerably upon weight bearing activity, with over-pronation (flexible flat feet) Tenderness maximum exactly as in the picture The voluntary range of motion of the foot and ankle mortice were preserved
