PROCESS Series of 4 modules and questions on Etiologies, Evaluation, & Management Step #1 Power point module with voice overlay Step #2 Case-based question and answer Step #3 Proceed to additional modules or take a break
Slide 3-

Objectives Upon completion, the learner will be able to: List and visually identify the normal age related skin changes Identify and treat the common skin disorders of aging Identify malignant versus non-malignant skin conditions in aged
Slide 4-

Learner’s options: Review techniques to improve dermatologic diagnosis or Skip to “Skin Show" (proceed to slide 12)
Slide 5-

Dermatologic diagnosis “wizardry” “Augenblinck”: The experienced make dermatologic diagnosis in the first 5 seconds of viewing. How do they do it? See next slide
Slide 6-

Steps to improve dermatologic diagnosis Step #1: Describe the physical characteristics of the lesion/lesions Step #2: Note the location Step #3: Look for associated lesions Step #4: Develop a differential Remember: “Common things happen commonly with varying presentations”
Slide 7-

Describe these lesions: Terms you might have used: -macular -pigmented -smooth -variable color -irregular margins -indistinct margins Location: arm Associated lesions: some on other arm
Slide 8-

What is the differential diagnosis? A) malignant melanoma B) senile purpura C) seborrheic keratoses What are the answers? Foreground: senile purpura Background seborrheic keratoses
Slide 9-

Describe these lesions:
Slide 10-

Describe these lesions: Terms you might have used -macular and papular -confluent and discrete -temperature -distribution -blanching? Location: inter-trigonal Associated lesions: Other inter-trigonal areas
Slide 11-

What is the differential diagnosis? Shingles? Why or why not? Cellulitis? Why or why not? Candida? Why or why not? Allergic reaction? Why or why not? Answer: Candida
Slide 12-

EPIDERMAL AGING In youth, epidermis interdigitates with dermis With aging, the interdigitations flatten, resulting in: reduced contact between epidermis and dermis decreased nutrient transfer increased skin fragility
Slide 13-

LIPIDS AND AGING Aging is associated with decreased lipids in top skin layer, which leads to: Dryness and roughness Decreased barrier function
Slide 14-

IMPAIRED HEALING AND IMMUNE FUNCTION WITH AGING Slower turnover of epidermal cells may account for slower rate of wound healing Lower number of immune antigen-presenting cells (such as Langerhans) may cause reduced cutaneous immune surveillance
Slide 15-

CHANGES IN THE AGING DERMIS Decrease in ground substance leads to wrinkling, atrophy Decrease in collagen and elastin cause haphazard, fragmented fiber orientation

Post-test A 72-year-old woman who is concerned about her facial skin inquires about the use of tretinoin to make her “look younger.” Which of the following clinical findings is most consistent with physiologic aging, as opposed to photoaging, and would be more reversible with the use of tretinoin?
Slide 18-

Which of the following clinical findings is most consistent with physiologic aging, as opposed to photoaging, and would be more reversible with the use of tretinoin? Localized telangiectasias Generalized loss of elasticity Localized hypopigmentation Generalized xerosis Localized hyperpigmentation Used with permission from: Murphy JB, et. al. Case Based Geriatrics Review: 500 Questions and Critiques from the Geriatric Review Syllabus. AGS 2002 New York, NY.
Slide 19-

Answer: D. Generalized xerosis With normal aging, changes occur in the skin that are different from those of sun exposure, called photoaging. Photoaging, a result of chronic sun exposure, produces the vast majority of undesirable skin changes. Photoaged skin shows more telangiectasias and spotty hyperpigmentation and hypopigmentation. In addition, there is cellular dysplasia, epidermal disorganization, and marked elastosis in the dermis. A decrease in collagen content is noted, as well as perivascular inflammatory infiltrates. The end result is that the skin appears wrinkled, yellowed, rough, and leathery.
Slide 20-

Physiologic changes include a reduction in the number of melanocytes and immune cells, a decrease in the number and function of eccrine sweat glands, and a decrease in function of the apocrine sweat glands. The most marked physiologic change, however, is xerosis, or dry skin, a result of changes in the stratum corneum that include an increase in the time required for cellular renewal and a decrease in the moisture content of the layer. Tretinoin (all-trans-retinoic acid) has been shown to result in improvement in histologic appearance on non–sun-exposed protected areas of the skin in older patients. Reversible changes with topical daily applications have included thickening of the epidermis and rete ridges and normalization of the matrix. It is thought that the retinoids (eg, tretinoin) play a role in cell proliferation and deregulation of skin cell replication. end
