About This Presentation

Description:Primary care physicians (PCP) take care of the majority of children with medical complexity (CMC), but their role in care coordination is often unclear. Most families with a CMC look for a separate community or hospital-based solution for pediatric primary care to improve access to care. Primary care physicians (PCP) care for the majority of CMC.
