Understanding Gynecological Emergencies Gynecological emergencies refer to acute conditions related to the female reproductive system requiring immediate medical intervention. These emergencies can be life-threatening and require swift diagnosis and treatment. Common examples include ectopic pregnancy, ovarian torsion, and severe pelvic inflammatory disease (PID). Reference: American College of Obstetricians and Gynecologists. (2020). Emergent Gynecologic Conditions. Retrieved from www.acog.org.

Ectopic Pregnancy Occurs when a fertilized egg implants outside the uterus, most commonly in a fallopian tube. Symptoms: Sharp pelvic or abdominal pain, vaginal bleeding, dizziness, shoulder pain. Risk Factors: History of pelvic infections, previous ectopic pregnancy, and endometriosis. Diagnosis: Ultrasound and blood tests for hCG levels. Treatment: Medical (methotrexate) or surgical intervention (laparoscopy). Reference: Mayo Clinic. (2021). Ectopic Pregnancy: Symptoms, Causes, and Treatments. Retrieved from www.mayoclinic.org.

Ovarian Torsion A condition in which an ovary twists around the ligaments supporting it, cutting off its blood supply. Symptoms: Sudden, severe abdominal pain, nausea, vomiting. Risk Factors: Ovarian cysts, enlarged ovaries, and fertility treatments. Diagnosis: Ultrasound with Doppler to assess blood flow to the ovary. Treatment: Emergency surgery (laparoscopy) to untwist the ovary. Reference: Cleveland Clinic. (2021). Ovarian Torsion: Causes, Symptoms, and Treatment. Retrieved from www.clevelandclinic.org.

Acute Pelvic Inflammatory Disease (PID) PID is an infection of the female reproductive organs often caused by sexually transmitted infections (STIs). Symptoms: Lower abdominal pain, fever, unusual vaginal discharge, painful urination. Risk Factors: Multiple sexual partners, history of STIs, unprotected sex. Diagnosis: Pelvic exam, ultrasound, and testing for STIs. Treatment: Antibiotics, hospitalization in severe cases. Reference: Centers for Disease Control and Prevention (CDC). (2021). Pelvic Inflammatory Disease (PID): Symptoms and Treatment. Retrieved from www.cdc.gov.

Ruptured Ovarian Cyst An ovarian cyst can burst, leading to sudden, severe pain and internal bleeding. Symptoms: Sudden, sharp lower abdominal pain, bloating, and possible lightheadedness. Diagnosis: Ultrasound to identify cyst rupture; blood tests to check for bleeding. Treatment: Pain management, monitoring; surgery in severe cases of bleeding. Reference: American Academy of Family Physicians (AAFP). (2020). Ovarian Cysts and Their Complications. Retrieved from www.aafp.org.

Severe Menorrhagia Excessive menstrual bleeding that may require emergency care due to severe blood loss. Symptoms: Soaking through sanitary products every hour, prolonged bleeding, dizziness, fatigue. Causes: Fibroids, hormonal imbalance, bleeding disorders. Diagnosis: Pelvic ultrasound, blood tests for anemia, hormonal testing. Treatment: Hormonal therapy, antifibrinolytics, blood transfusion in severe cases. Reference: National Institutes of Health (NIH). (2021). Heavy Menstrual Bleeding: Diagnosis and Management. Retrieved from www.nih.gov.

Miscarriage and Associated Complications Miscarriage is the spontaneous loss of a pregnancy before the 20th week. Symptoms: Vaginal bleeding, abdominal pain, passing tissue or clots. Risk Factors: Advanced maternal age, previous miscarriage, underlying health conditions. Diagnosis: Ultrasound to confirm loss; blood tests for hormone levels. Management: Watchful waiting, medication, or surgical intervention (D&C). Reference: World Health Organization (WHO). (2020). Miscarriage and Pregnancy Loss: A Clinical Guide. Retrieved from www.who.int.

Uterine and Postpartum Hemorrhage Excessive bleeding from the uterus, often occurring after childbirth. Symptoms: Heavy vaginal bleeding, low blood pressure, rapid pulse, weakness. Causes: Uterine atony, retained placenta, lacerations. Diagnosis: Physical exam, ultrasound for retained tissue. Treatment: Uterotonics (e.g., oxytocin), blood transfusions, surgical intervention if necessary. Reference: Royal College of Obstetricians and Gynaecologists (RCOG). (2021). Postpartum Hemorrhage Management. Retrieved from www.rcog.org.uk.
Slide 11-

Emergency Contraception Used to prevent pregnancy after unprotected intercourse or contraceptive failure. Options: Oral emergency contraceptive pills, copper IUD. Time Frame: Effective within 72 hours (pills) to 5 days (IUD). Importance: Prevents unintended pregnancy; not a replacement for regular contraception. Reference: Planned Parenthood. (2021). Emergency Contraception Options and Efficacy. Retrieved from www.plannedparenthood.org.
